Your property located at 64 Cassell Rd. has a General NPDES permit with the State of North Carolina. This permit
is for a Single Family Residence (SFR) wastewater treatment system that discharges into a waters of the State. As
owners, you are responsible for maintaining this system in compliance with the current NCG550000 permit. A copy
of this. permit can be found.online at htt de .nc. ov about divisions water -resources water -resources -
perm its/wastewater-branch/npdes-wastewater/general-permits.
The North Carolina Division of Water Resources conducted a Compliance Evaluation Inspection of the SFR located
at 64 Cassell Road on December 13, 2016. This inspection was conducted to verify that the facility is operating in
compliance with the conditions and limitations specified in NPDES WW Permit No. NCG550692. A summary of the
findings and comments noted during the. inspection are provided in the enclosed copy of the inspection report.
The Compliance Evaluation Inspection was conducted by Division of Water Resources staff from the Asheville
Regional. Office (ARO). The following deficiencies were noted during the inspection:

Septic Tank Septic tank maintenance records were not available at the time of the inspection.
[NCG55.0000 Part I. A. 3. Permit Conditions (Operations & Maintenance) Septic
tanks shall be inspected at least yearly to determine if solids must be removed or
if other maintenance is necessary. Septic tanks shall be pumped out every five
years or when the solids level is found to be more than 1/3 of the liquid depth in
any compartment.]
Corrective Measures for the deficiencies noted above:
Permit: Please remit payment and submit Name/Change of Ownership Form to Raleigh.
Effluent Sampling: Sample or document no discharge and submit to ARO.
Septic Tank: Submit septic tank maintenance and/or pumping records to ARO.
In addition, the issue below must also be addressed:
Compliance Issue:
It was noted during the inspection that the chlorination tubes are checked once in a while, but not consistently.
The chlorination tubes should be checked at regular intervals and restocked with chlorine tablets (rated for
wastewater) as needed.
Remedial actions should have already been taken to correct this problem and prevent further occurrences in the
future. The Division of Water Resources may pursue enforcement action for this and any additional violations of
State law.
To prevent further action, please respond in writing to this office within 30 days upon receipt of this Notice
regarding your pians or measures to be taken to address the indicated deficiencies and compiiance issues.

Inspectors, Mikal Willmer and Tim Heim with the Asheville Regional Office conducted a Compliance
Evaluation Inspection (CEI) of the Single Family Residence (SFR) wastewater treatment system
located at 64 Cassell Rd on December 13, 2016. Inspectors were unable to contact and meet onsite
with the current owner. The listed permittee's daughters are now trustees. This property is currently for
sale. Inspectors did speak with the current renter about the property.
It is believed the septic system was last pumped five years ago. The property is only used
occasionally. There are no permanent residents. This is a commercial property. Chlorine tablets are
added to the chlorination tubes on occasion, but are not checked at consistent intervals. Annual
sampling and septic tank maintenance records were not available at the time of the inspection. System
was not discharging during the inspection. When the property is sold, a Name/Ownership Change
Form should be given to the buyers to complete and submit to Raleigh.

Comment: There are no full time residents at this property. A renter uses the shop on occasion and
restocks tablets every now and then. Inspectors asked the renter to check the chlorine
