Stormwater Permit No. SW3201001

Dear Mr. Church:
The Division of Energy, Mineral and Land Resources (DEMLR), received a Stormwater
Management Permit Application for the subject project on October 6, 2020. A review of that
information has determined that the application is not complete. The following information is
needed to continue the stormwater review:
Please provide a signed and notarized O&M Agreement and Plan [15A
NCAC .1042(2)0)]. The O&M EZ form can be found on the Stormwater Design Manual
wehsite_
2. Please provide a USGS map identifying the location and the GPS coordinates for the
project [15A NCAC 02H .1042(2)(d)].
3. Please provide a copy of the property deed to verify ownership of the parcel.
4. Sand Filter MDC 1 requires the separation from the bottom of the sand filter to the
seasonal high water table to be at least two feet [15A NCAC 02H .1056(1)]. Page 5 of the
submitted geotechnical investigation states that the depth to groundwater at boring B-9,
which is in the footprint of the SCM, is two feet. Based on the existing grading shown on
sheet C200, the existing ground elevation is approximately 775' in the area of the
proposed SCM. Therefore, the groundwater elevation is approximately 773' as opposed
to 769' as shown in the Supplement EZ form. This does not provide sufficient separation
from the bottom of the sand filter to the seasonal high water table.
5. Section View `A' on Sheet C902, Detail 1 shows the bottom of the system at El. 771.50.
Detail 2 shows the invert of the pipe to be 771 and the bottom of the concrete to be
770.50. Please correct this discrepancy.

6. Detail 2 on Sheet C902 shows the invert leaving the outlet structure at El. 771.10, which
is 0.10' above the bottom of the outlet structure. The invert of the outlet should be even
with the bottom of the outlet structure to prevent standing water in the outlet structure.
7. Provide pdfs of all revisions, 2 hardcopies of revised plan sheets, and 1 hardcopy of other
documents. Pdfs must be uploaded using the form at:

Please note that this request for additional information is in response to a preliminary review.
The requested information should be received by this Office prior to January 1, 2021, or
the application will be returned as incomplete. The return of a project will necessitate
resubmittal of all required items, including the application fee. Please reference the State
assigned project number SW3201001 on all correspondence.
If you need additional time to submit the information, please submit your request for a time
extension to the Division at the contact below. The request must indicate the date by which you
expect to submit the required information. The Division is allowed 90 days from the receipt of
a completed application to issue the permit.
The construction of any impervious surfaces, other than a construction entrance under an
approved Sedimentation Erosion Control Plan, is a violation of NCGS 143-215.1 and is subject
to enforcement action pursuant to NCGS 143-215.6A.
