On September 5, 2019, Mooresville Regional Office (MRO) staff completed the Compliance Inspection for
the above -stated permit. The inspection included a review of the Application Fields, Spill Plan and O&M
Records. A review of the material loading area at the Fourth Creek WWTP was also performed as part of
the inspection. The MRO would like to thank Alex Fox of Synagro for his time during this inspection.
Please email a copy of the Laboratory TCLP and PAN results once the data has been received for the
residual material as part of the inspection. The fields were in good condition and the material appeared to
be being evenly applied across the application area.

Inspection Summary
A Land Application event is scheduled to occur on the Dennis Phillips NC-IR-20, fields 8, 9,1 & 2, under permit WQ0016247
Synagro Western Piedmont RLAP using Class "B" residual from the City of Statesville 3rd and 4th Creek WWTPs. There
are no limiting slopes present on the any of these fields scheduled for land application.
Mention was made that haul trucks transporting the residual cake to the LA fields will need to employ their retractable
covers to prevent material from leaving the trucks while in route to the LA fields.
A rain gauge was not observed during the site visit. Please be sure that a rain gauge is installed to measure precipitation
during the LA event.

Comment: A copy of the permit was present during the site visit. A copy of the spill plan and O&M plan
were also viewed during the site visit. All application haul trucks will have a copy of the permit
and the spill plan present in the vehicles during the LA event. Monitoring Wells are not required
as part of this permit.
Soil samples were collected before LA event. There appears to be adequate separation
between the surface and the top of groundwater.
The fields appeared to be in good condition. All animals have been removed from the fields
where residuals are scheduled to be applied. MRO staff observed the fields being buffered. All
surface water and drainage features were adequately buffered to avoid residual material from
entering any onsite surface water features.
PAN was not reviewed during the site visit. TCLP analytical data should be forwarded to the
MRO upon receipt of the results.

2017, which included the facility storage/loading areas and active application at field NC-ST-03-
04. Residuals management at the facility and the field site appeared to meet program
requirements at the time of inspection.

which need preventative spill protection during all loading events and is the responsibility
of SYNAGRO, since they are the permittee in this case. MRO staff noted this concern
during the inspection and Mr. Zufall placed covers over the grates.

Per conversation with Andy Smith, the City of Statesville has increased the residuals
"SOUR" testing to once/month to help address odor and vector concerns noted in a
previous inspection and to ensure VAR is met, particularly with the Third Creek residuals,
which typically have more odors than the Fourth Creek residuals. The once/month
sampling exceeds the permit requirement; therefore, the City of Statesville may reduce
sampling frequency back to once/quarter at their discretion.

This inspection included an active land application (LA), of Third Creek residuals at field NC-IR-20-03,
under the Synagro — WQ0016247 permit; and, a review of records and residuals storage at Statesville's
Fourth Creek WWTP, for both of their permits referenced above. The 4th creek plant was undergoing
construction for repairs at the lime processing and storage areas, at the time of this visit. Statesville
maintains separate bay areas for the "class A" residuals storage and distribution, and the 3rd creek and 4th
creek "class B" residuals handled by SYNAGRO.
o Strong odors were noticed in the Class B residuals at the field LA site; however, vectors
(flies) were minimal. Statesville has increased sampling frequency for SOUR testing to
ensure VAR is met. The once/month testing is at the discretion of the City of Statesville and
may be reduced to the permit requirement of once/quarter at any time.

conducted on March 4, 2013. 1 would like to thank Andy Smith for taking the time to show me around
the Statesville 3rd Creek Wastewater Treatment Plant (WWTP), 4th Creek WWTP and Water
Treatment Plant (WTP) facilities.
The report indicates compliance, at the time of inspection, for all three permits, but note
under records keeping the need to complete all portions of the "Alkaline Stabilization"

This inspection included the 3rd Creek WWTP, 4th Creek WWTP and Statesville WTP facilities, and
covers all three City of Statesville permits: #WQ0006245 (Class A), WQ0014543 (Emergency LA - not
used) and WQ0016247 (Class B - Synagro LA).
The 4th creek plant was the only plant actively processing sludge (dewatering), at the time of inspection,
and 4th creek is where all residuals are stored. There are separate bay areas for the "class A" residuals
storage and distribution, and the 3rd creek and 4 h creek "class B" residuals handled by SYNAGRO. No
unusual odors or vectors were noticed in any of the residuals areas (sludge dewatering or storage) and all
areas of all three plants were neat and clean in appearance.
The Mooresville Regional Office (MRO) noted a couple of record keeping issues.: The Alkaline
Stabilization records showed some deficiencies, as not all data was recorded or dates and times were
missing. MRO discussed the importance with Andy Smith, as these records are their proof of
Pathogen and Vector attraction reduction, for the Class A residuals (WQ0006245), and the "Annual
LA Cert Form" for WQ0014543 did not include an explanation of how residuals were handled:
There was no land application to this emergency use permit. This should be stated on future forms.

transported, and stored at the Fourth Creek WWTP. Storage at the Fourth Creek plant
appeared to be managed appropriately. An inspection of the plant by surface Water noticed
some housekeeping issues with storage last month and suggested adding higher retaining
walls to the area. This area has been improved with one layer of cement block. This appears
to be working appropriately. Plant management will continue to add block to the area as
needed.

The City of Statesville uses permit WQ0014543 as an emergency alternative for Third Creek and Forth
Creek WWTP's. This permit has not been used since the permit was originally issued (1998).
SYNGRO holds permit WQ0016247 exclusively for City of Statesville's class B land application
program. The City has a Marketing and Distribution Permit (class A lime stabilization WQ0006245)
that is the City's other "end use" option for the sludge produced at the Third Creek and Fourth Creek
WWTPs. Records reflect compliance with all three of the permits listed above. There is no sludge
storage at the Third Creek plant. Sludge is de -watered at the plant and stored at the Fourth Creek
WWTP. A new sludge belt Press replaced the old plate and frame dewatering equipment and was

Please consider this a formal notification of Synagro's plans to land apply residuals from the
Town of Statesville's WWTP onto sites owned by Dennis Phillips. This is the first land
application event to this farm, since the land was added to the program as of 5/08/2007.
This notification is in accordance with the Town of Synagro's Land Application of Residual
Solids Permit No. WQ0016247, issued by NC DFNR/DWQ. Section 1,10, states "...the
appropriate local governmental official (i.e., county manager) shall be notified at least 24
hours prior to the initial residuals land application event on any new site so that they will be
aware that residuals land application activities have commenced on the site".
