The facility was found to be non -compliant with the Deemed Collection System requirements. The requirements for your sewerage collection system can be found in 15A NCAC 02T .0403, Permitting By Regulation. These regulations place significant operation, maintenance and reporting requirements on those entities that own or operate a wastewater collection system with average daily flows of less than 200,000 gallons per day. I have included a copy of the regulations for your reference. The following items were deficient at the time of the inspection: 1. The telemetry system at the lift station was not fully functional. The float valve and visual alarm was not working. Also, the coupling to pump #1 was leaking and should be repaired. 2. The manhole at 310 Wendy Knoll had an excessive amount of grease. If you have any questions concerning the regulations or those items that were deficient, please do not hesitate to contact me. The owner has developed an Operation & Maintnenace Plan, Spill Response Plan with emergency contact numbers and equipment list. Grease control information will go out to homeowners in the August. High priority lines are checked every 6 months - last inspection entry was 3/7/14. Lift station is checked every Monday per maintenance log. The telemetry system at the lift station was not working at the time of the inspection. The visual alarm and float valve were not working. A pipe coupling to pump #1 was leaking. The manhole at 310 Wendy Knoll had an excessive amount of grease. According to a letter in file dated August 24, 2000, and the 2007 inspection notes, there were seven residences that were gravity flow to the force main. According to the ORC, there are still 5 residences that remain tied to the force main via gravity sewer. These remaining homeowners should construct individual orjoint pump stations to alleviate potential problems. The system consists of 1,150 feet of 8-inch diameter gravity sewer and 1,100 linear feet of force main. The upper section of the development is gravity. There are seven gravity taps to the force main. Tom has developed an "Operational Plan", a system map, inspects the telemetry served pump station weekly, inspects annually the portion of the gravity line not routinely seen by residents, and includes advice on proper disposal of residential grease in the annual report going out to each resident. The system has no high -priority lines nor does it have commercial sources of cooking grease. Operation and maintenance of the system is in compliance with the requirements of 15A NCAC 02T.0403. These Regulations place significant operation, maintenance and reporting requirements on those entities that own or operate a wastewater collection system with average daily flows of less than 200,000 gallons per day. These regulations are applicable to your facility. This letter is provided as guidance to assist you in complying with the new reporting and operations and maintenance (O&M) requirements and to advise you that you are subject to system review, inspections and possible enforcement, if the system is not in compliance with the regulation. Per G.S. 143-215.1 C(b), the responsible party of a discharge of 1,000 gallons or more of untreated wastewater to surface waters shall issue a press release within 48-hours of first knowledge to all print and electronic news media providing general coverage in the county where the discharge occurred. When 1.5,000 gallons or more of untreated wastewater enters surface waters, a public notice shall be published within 10 days and proof of publication shall be provided to the Division within 30 days. Refer to the referenced statute for further detail. The Director Division of Water Quality, may take enforcement action for SSOs that are required to be reported to Division unless it is demonstrated that: 1) the discharge was caused by severe natural conditions and there were no feasible alternatives to the discharge; or 2) the discharge was exceptional, unintentional, temporary and caused by factors beyond the reasonable control of the Permittee and/or owner, and the discharge could not have been prevented by the exercise of reasonable control. Part II must be completed to provide a justification claim for either of the above situations. This information will be the basis for the determination of any enforcement action. DYes Auto-dialer/telemetry (one-way communication) Page 4 CS-SSO Form October 9, 2003 Mountain Valley SSO - Chronology of events: (times are approximate) TUE 02-05-08 1000: Got a phone call report of a broken manhole on Pump House Road in Mountain Valley Subdivision; new construction area with very new driveway grading work; found manhole flowing at lid rim base - possible construction damage; manhole is full - not broken; rim grout is cracked; soft soil shows evidence of overflow; overflow flows onto a wooded area. Checked lift station - three quarters full, pumps running bit not moving any water; Assumed impellers partially clogged (surmised that recent 2 inch and subsequent 1 inch rain events in the past 4 day period flushed collection line debris into lift station; I/I has not been a problem on this system but there is one manhole located in a ditch line). Toggled pumps off and on several times. Cycling pumps appears to have relieved problem - pumps now moving water. Called for pump maintenance. 1600: Maintenance crew reports pumps running and appear normal. WED 02-06-08 1000: WWTP influent flow past 24 hrs. - .008 mgd; situation appears normal. Total flow is as expected (average daily flow is .005 gpd); Lift station is pumped down. 1600: Got a call from resident of audible and visual alarm at lift station. Note: Auto dialer did not call with high water alarm condition; Found pumps running but not moving much water; Lift station is 3/4 full; The assumption is that the force main has a partial blockage (this line has a sag point under the highway and has a history of blockages; it gets flushed as needed). Called Roto- Rooter. Roto-Rooter could not come until next morning. Left pumps running; manhole not overflowing; got no calls overnight of slow or backed up sewers. Checked auto -dialer - not functioning. Will alert maintenance. THUR 02-07-08 1040: WWTP influent flow - .003 mgd; Lift station full; Manhole overflowing - estimated 1 gallon per minute; flow is .002 mgd lower than average daily flow. 1130: Roto-Rooter flushed force main; exposed only minor debris but action appears to have cleared the blockage. Pumps discharging but appear to have slightly lower than normal discharge volume?? 1300: Pumps running; discharge volume still appears to be lower than normal??? Called pump maintenance. Maintenance crew scheduled for Friday - am. 1515: Phoned DENR to report overflow situation. FRI 02-08-08 1030: Pump Maintenance Crew arrives on site; checked both pumps - both pumping. Septage pump truck arrives to pump lift station; After some puzzled discussion, maintenance crew cycled pumps off and on. This time, pumps did not move water??? #1 pump will pressurize the discharge line but will not move water to the WWTP; Maintenance opinion is that force main is still blocked. HOWEVER, after additional puzzled discussion, maintenance man ran a hooked "fish tape" thru the discharge valve and discovered a mass of rags and plastic debris hung in the discharge valve. The mass was approximately 1.5 inches x 6 inches long. This mass appears to have acted in a fluctuating manner - at times allowing water to pass and at other times no water would pass thru the line. It was an unusual situation. Blockage was cleared and pump placed on line by 1130 am. 1130: #2 pump clogged; pulled pump, removed a rock and plastic debris from impeller; pump placed in service. 1200: Pump truck removes sand and other debris from bottom of lift station. Maintenance crew checks auto -dialer; auto -dialer has no dial tone; called AT&T for line repair service. 1600: AT&T repairs phone line; Auto -dialer functional and back in service. Information available to me indicates that flows, and therefore velocities, through the line are insufficient to keep the line scoured clean in low places. The introduction of sewage from seven residences by gravity into a force main defies all engineering logic. Under the best of circumstances pump energy is wasted pushing sewage up one or more of the seven gravity lines. This pump energy would otherwise be available to keep velocities in the line up. Under the worst'of circumstances, when the force main is partially or completely clogged, sewage is. forced back up the gravity lines above the clog with sometimes disastrous results. It gives me no pleasure to tell you that, in my opinion, the best solution is for the owners of the seven gravity connections to the force main to construct individual or joint sewage pumping stations which would discharge into the force main. Pumping is the only.logical way to introduce sewage into a force main. When the force main is clogged sewage cannot be backed past the individual pump stations. I have to believe that individual pump stations would result in generally higher velocities in the force main thereby helping to reduce the accumulation of material which could cause blockages. Experience tells us that the system of flapper valves does not work. Thank you for your response to our notification of operational difficulties with the sewerage system of the Mountain Valley Subdivision. Brief History: Originally all homes were on a septic tank with leach bed system. Eleven or twelve years ago, in anticipation of building homes further up the mountain that would not have soil for leach beds, a grinder - pump station and a disposal unit was installed to accept raw sewerage, process it and discharge the treated water into the French Broad River. At that time there were houses still being built that would be below the grinder station. Options were offered to the owners of these homes. They could have a leach bed or be connected to the sewer line. Seven such homes were connected. There has been a 10 year history of problems of errors of judgement or expertise causing the sewer line 3 times to become inoperative. However, these were mistakes and not due to the design of the system. While costly and annoying to the septic tank victims the problems were handled by Dan Ducote. The system was eventually sold to the Culligan Water Treatment Company for $ 1.00. Now, however there has been two recent problems indication that the system will not operate without periodically being lugged with sediment gathering in the lower section of the sewer pipe. The sewer line was originally planned to follow the easement along Danielle Drive and then to follow the lot lines down to the easement along Cummins Rd. to the disposal unit. However, because it evidently was more expedient, the line was run down through the middle of the back yard of 153 Mountain Valley Drive and 145 Mountain Drive. We do not know whether the line follows the lot lines between 149 Mountain Valley Drive and 136 Mountain Valley Drive. Current corrective action taken: After the third time the back yard of 153 Mountain Valley Drive was dug up to correct problems, I requested a clean out valve be installed. This has been opened to relieve pressure when the line is plugged with the sewerage then running into the nearby storin sewer drain which empties into the French Broad River. After the second stoppage this year Bergen Smith had a clean out installed in his back yard at 145 Mountain Valley Drive. This permits an extended coverage of the reverse flow Roto-Rooter snake to clear the line. These are only band -aid corrections. It is the opinion of the experts (?) that there will continue to be stoppages because the seven septic systems act as a sponge reducing the pressure supplied by the grinder system allowing silt to stop the line. Cost would be abou a.thousand dbll`ars pei` mstallarion. Of course the full amount would be at the homeowner's expense. Neither Culligan nor Blue Mountain has ever reimbursed any of the expenses incurred by the homeowners due to the back-up problems. This proposal is not acceptable because: This will not guarantee that the line will not be plugged. Even with a pressure system the discharge from the house will back up. This will back up in the house bathtubs and toilets and cause the septic tank to overflow. 2. Improved type back flow flapper valve protection. This proposal is not acceptable because: Flapper valves, regardless of type, can be jammed by material not completely pulverized by the grinder. Back - flows into my septic tank from the grinder have included condoms, sanitary napkins and other solids. Despite having to pay additional costs for septic tank maintenance (This last back-up septic cost was $350.00) we are required to pay the same fee as those without septic tanks. 3. Build leach beds for those with septic tanks. This is a possible solution for those with adequate space. In my situation I have enough space in my back yard but some may not. However, if I install the leach bed then I will cap the sewer- line and a new line will have to be installed following the original maps. There is no variance or easement for the line to be run through my yard. 4. Final Proposal: This proposal comes from no specific source. It may have been a quote from Poor Richarson's Almanac or something I read in Don Quotte. " If you don't like the way the Mountain is -run, go -back to where you -come from." This proposal is also not acceptable because in all good conscious, I could not sell my property unless this problem had been adequately solved. My Proposal: 1. Where the sewer line makes the 90-degree turn to follow Cummins Rd. to the disposal unit, install an old- fashioned manhole or collector with a warning device to indicate a back up. Install at this point a pressure pump to prevent the accumulation or residue in the lower section of the sewer line. (or) 2. Install clean -outs at no less than ever hundred feet and at and turn of more than 30 degrees. Then, using the available video system, scan the line. and locate those sections, that are causing a trap in the line for residue and replace them.
