Dear Mr. Hills:
The Division of Energy, Mineral and Land Resources (DEMLR) received a Stormwater Management
Permit Application for the subject project on October 23, 2020. A preliminary review of the application
has determined that additional information is needed to meet the requirements of 15A NCAC 02H .1000
and continue the stormwater permit review. Failure to provide the following additional information to
DEMLR by December 24, 2020 will result in the application being returned as incomplete. The
return of a project will necessitate resubmittal of all required items, including the application fee.
1. Please provide documentation from the North Carolina Secretary of State showing that the LLC is
an active corporation in good standing with State of North Carolina [ 15A NCAC 02H
.1045(2)(b)(i)].
2. Please provide documentation from the NC Secretary of State or other official documentation
showing the titles and positions held by person that signed the application [ 15A NCAC 02H
.1045(2)(b)(ii)].
3. The individual signing Part G: Certification of Applicant/Financially Responsible Party must
print their name on the first line.
4. The Design Professional's Certification section was not filled out. This is required to issue the
permit [15A NCAC 02H .1043(3)(d)]. Please note that Fast -Track Projects are not reviewed for
technical compliance before issuing the Authorization to Construct Permit. A licensed
professional who is responsible for ensuring that the project complies with the MDC must always
be kept on file and oversee the project. The project will be reviewed for technical compliance
upon completion of the project. The financially responsible party will be responsible for making
any changes or facing disciplinary action if necessary.
