Subject: Request for Additional Information
Stormwater Permit No. SW4200902
Miller's Reserve
Forsyth County
Dear Mr. Bryant:
The Division of Energy, Mineral and Land Resources (DEMLR), received a Stormwater
Management Permit Application for the subject project on September 22, 2020. A review of that
information has determined that the application is not complete. The following information is
needed to continue the stormwater review:
1. The table in Part IV.10 of the SWU-101 application must be in square feet, not acres.
2. The BUA in the above referenced table for on -site buildings/lots is less than the amount
allowed by the deed restrictions if each lot is fully built out.
3. Please submit a short narrative describing the project and the stormwater treatment
system. In the narrative either include the calculations separately or a note stating that the
calculations are on the plan sheets. Please also make note that there is existing BUA and
only the net increase is required to be treated in the SCM.
4. Please submit a drainage plan showing the location of the stormwater collection system
including the locations of inlets, outlets, pipes, and swales, as well as the inverts and
diameters of pipes excluding driveway culverts [ 15A NCAC 02H .1042(2)(g)(vii)].
5. Please show a maintenance access and easement that meets the requirements of General
MDC8&9.
6. The average depth and orifice diameter specified on the Supplement EZ form don't
match the calculations and details shown on the plans (Supplement Fields 28 and 37,
respectively).
7. Please re -upload the entire plan set at the link below.

Please note that this request for additional information is in response to a preliminary review.
The requested information should be received by this Office prior to November 19, 2020, or
the application will be returned as incomplete. The return of a project will necessitate
resubmittal of all required items, including the application fee. Please reference the State
assigned project number SW4200902 on all correspondence.
If you need additional time to submit the information, please submit your request for a time
extension to the Division at the contact below. The request must indicate the date by which you
