Hello Gents,
have attached the Injection Event Record for the subject permit. The well installation
records for IW-1 and IW-3 have already been submitted to the DENR.
This is the 5th injection event at the site and was performed in April 2015. As I
discussed with Michael yesterday, injection volumes slightly exceeded our target of
25,000 gallons per well (by 1,000 and 496 gallons) due to the batching process. I
understand that the DENR does not consider the slight overage an issue.
Thirty (30) day monitoring in both injection areas will be performed on or about May

Hello Michael, Andrew, and Landon,
This email is notice of intent to inject milk solution into injection wells IW-1 and IW-3 at
the former Andrex property (UIC Permit W10100106) beginning April 6, 2015.
Injection will be performed in accordance with the Permit and the DENR approved
2015 Injection Plan. An Injection Event Record will be prepared and submitted as
required after the injection event is complete.
Actual start date and time will be dependent upon milk solution availability and
weather. DENR inquiry into the up to date project schedule should be made via
telephone to Ben Nisbeth of BLE at 828-545-3414. It's anticipated that up to 2 weeks
will be required to complete the injection event.
This detailed email notice is made as a substitute for the verbal notice specified in the
permit. If this email notice is not sufficient, please contact me and verbal notice will be
provided.
