Kimley-Horn and Associates, on behalf of the City of Henderson, would like to request a
Permit Application Correction to the joint 404/401 Preconstruction Notification
Application submitted on July 1, 2010 for the above reference project. Prior to
submitting the application package, NCDOT's Division Environmental Department and
Hydraulics Unit reviewed the drainage plan and made a few minor comments. Their
comments resulted in modifications to both stream crossings/impacts. These
modifications were reflected in the permit drawings submitted with the application, but
did not get updated on sheet 5 of the Pre-Construction Notification Application Form.
The correct stream impacts are as follows:
• 26 linear feet of intermittent stream impact (original application indicated 21 If)
• 89 linear feet of perennial stream impact (original application indicated 59 If)
The revised total stream impact is 115 linear feet which still falls below 150 linear feet
threshold and results in no anticipated mitigation.
Per your conversation with Beth Reed on July 19, 2010 we are sending you 5 copies of
revised sheet 5 of the PCN to replace in the original application package.
