Hey Dave,
Thanks for running through the R-2915 A and B projects with us. I have highlighted the issues that we need to address
below. I'll follow up with photos of those sites to you and Lori, once the contractor has fixed them. Also, thanks for
noting the NPDES issues. I'll share with our Div 11 roadside environmental staff next time I see them. We'II also be sure
and keep you in the loop regarding any stormwater changes on future projects. As always, I appreciate you getting back
to us so quickly on those possible alterations.
I'll be back in touch later this winter, and hopefully we can schedule a walk through of the R-2915 C section. I'll try and
find a less windy day this time!

Hey Kevin,
Below is a summary of my notes from our site visits (on a very windy day) with Ivan and Brandon along with a
few attached pictures.
Section B
• Site 1A culvert inlet area. Reinstall ground cover on hill slope where equipment disturbed existing
vegetation. Remove the traffic cone from the stream channel and the pile of 57-stone from riparian
area. High and low flow barrels appeared to be functioning well.
• Site 2 pipe outlet. Area appears to be stable.
• Sites 9, 17 and 18 pipe outlets and rip rap appeared to be stable. Some 57-stone needed to be
removed from bank near outlet at Site 18.
• NPDES records were in order. As a reminder, a DOT staff inspector for the project needs to sign the
forms along with the contractor performing the inspections. And, remind inspectors to enter the date
when repairs to erosion control measures were completed. If inspectors cannot make repairs within
the 24-hour or 5-day requirements, a note needs to be made providing reasons why (e.g., area was not
accessible due to current flooding or unsafe conditions, etc.)
Section A
• Outlets at Sites 24, 23 and 17 appeared to be functioning with no instability noted.
• Outlet at Site 16. Riprap placed for bank stabilization was washed out into Gap Creek following a storm
event. We discussed repair options and agreed on repositioning the rip -rap along the ditch outlet,
tying it into existing stream banks matching existing natural downstream aggradational bar elevation
(picture attached).
• Site 14 outlet. Rip rap along bank was stable.
• Site 11 culvert outlet. High and low flow barrels were functioning and stable.
• Site 5 outlet was functioning.
• Site 4 rip rap was stable and energy dissipator was stable and functioning.
• Our discussion with Ivan concerning his questions relative to stormwater management plans (SMPs)
was to clarify that any revisions to a SMP associated with a written 401 permit should be channeled
through you so that you can notify DWR.
Let me know if I missed anything or misinterpreted our conversations and how DOT decides to make repairs
relative to Site 16 on the A Section. Thanks.
