Dear Mr. and Mrs. Lopinski:
This Office has reviewed your CAMA application dated June 25, 2010 for the fill of 404 wetlands
beneath and adjacent to your house. The DWQ has determined that your application was incomplete as
discussed below. The DWQ will require additional information in order to process your application to
impact protected wetlands and/or streams on the subject property. Therefore, unless we receive the
additional information requested below, we will have to move toward denial of your application as
required by 15A NCAC 21-1.0506 and will place this project on hold as incomplete until we receive this
additional information. Please provide all of the following information so that we may continue to
review your project.
1. The DWQ requests that you avoid additional impacts to the 404 wetlands beyond the house
footprint. You must show that you have avoided and/or minimized to the greatest amount
practicable. For instance, instead of having the driveway come to the right side of the house,
have it go directly to/under the house. Parking can be located underneath the house and the
area shown as gravel driveway on the drawing could be greatly reduce and additional
wetland impacts could be avoided.
2. You have shown an infiltration trench partly located in the wetlands and this impact has not
been included in the application. Even if the impacts are temporary, you will still need to
include all impacts with the application.
3. You will also need to get a stormwater permit from the Division of Water Quality (DWQ).

Please respond within three weeks of the date of this letter by sending this information to me in writing
and two copies to Ian McMillan c/o 401 Wetlands Unit, 2321 Crabtree Blvd, Raleigh, NC 27604-2260.
Please reference the DWQ Project # of 2010 0509 in all future correspondence. If we do not hear from
you within three weeks, we will assume that you no longer want to pursue this project and we will
consider the project as withdrawn.
This letter only addresses the application review and does not authorize any impacts to wetlands, waters
or protected buffers. Please be aware that these impacts requested within your application are not
authorized (at this time) by the DWQ. If you have any questions, please feel free to call me at 910
