On behalf of Old North Utility Services, Inc. (ONUS), Land Management Group, Inc. (LMG) is
providing additional information regarding the above-referenced project in response to your letter
dated June 25, 2010 and in consideration of the Section 404 authorization recently received from
the US Army Corps of Engineers (USACE) Wilmington District Regulatory Branch.
The USACE has issued a Nationwide Permit Number 12 (see attached copy) authorizing the
proposed sewer main crossings and associated disturbances to jurisdictional wetlands and waters.
On behalf of ONUS, LMG is requesting that NC DWQ process the requested activities under Water
Quality Certification No. 3819 (for projects eligible for a Nationwide Permit Number 12). It is
understood that written approval from your office will be required due to the 0.008 acres of
permanent disturbance proposed. Accordingly, an application fee of $240 for "minor water quality
applications", is enclosed.
The proposed project meets the conditions of the General Certification for utility line activities and
qualifies for an NPDES General stormwater permit (NCGO10000). Five copies of this letter (with
attachment) are being provided for your use. Should you require any additional information
specific to the proposed sewer main upgrades, please do not hesitate to contact me either by email

Area #1 132.3 linear feet of 42 inch DTP sewer main crossing temporarily impacting 0.06 acre of riparian
wetlands.
Area #2 21.2 linear feet of 42 inch DIP sewer main crossing with a 21-foot by 20-foot by 6-inch concrete slab
permanently impacting 20 linear feet of stream channel and 0.01 acre of riparian wetlands.
Area #3 11.1 linear feet of 24 inch HDPE sewer main encased in a 36-inch steel pipe aerial crossing temporarily
impactin 30 linear feet of stream channel.

Your work is authorized by the above referenced permit provided it is accomplished in strict accordance with the attached
conditions and your submitted plans. Any violation of the attached conditions or deviation from your submitted plans may subject the
permittee to a stop work order, a restoration order and/or appropriate legal action.
This verification is valid until the nationwide permit is modified, reissued or revoked. All of the existing nationwide permits are
scheduled to be modified, reissued, or revoked prior to March 18, 2012. It is incumbent upon you to remain informed of the changes
to the nationwide permits. We will issue a public notice when the nationwide permits are reissued. Furthermore, if you commence
(i.e., are under construction) or are under contract to commence before the date that the relevant nationwide permit is modified or
refoked, you will have twelve (12) months from the date of the modification or revocation of the nationwide permit to complete the
activity under the present terms and conditions of this nationwide permit.
Activities subject to Section 404 (as indicated above) may also require an individual Section 401 Water Quality Certification, You
should contact the NC Division of Water Quality (telephone [phone]) to determine Section 401 requirements.
For activities occurring within the twenty coastal counties subject to regulation under the Coastal Area Management Act (CAMA),
prior to beginning work you must contact the N.C. Division of Coastal Management.
This Department of the Army verification does not relieve the permittee of the responsibility to obtain any other required Federal,
State or local approvals/permits.
