On July 13, 2010 the Division of Water Quality(DWQ)received your application set dated July 12,2010
For the above referenced project. The DWQ has determined that your application was incomplete and/or
provided inaccurate information as discussed below.
Additional Info-oration Requested:
® The 401 Certification cannot be processed until five(5)complete sets of the application and
associated maps are received at the DWQ Central Office in Raleigh along with the appropriate
fee. Please submit two(2) more complete application sets
Any large scale maps that are provided please include a copy of the site plans on CD. One(1)
data CD of full sized plans in TIFF Group 4 format(black and white, not grayscale or color).
if the plans are too large to store in TIFF formant,they can be stored in PDF.
You may also use FTP site and email them to bev.strickland<2ncdenr.ov Ms. Strickland can
be reached at(919) 733-1786 ifyou need further clarification.
® This project will require a permitting fee of$240 based on the reading of Water Quality
Certification 3819(NW 12). A check will be needed payable to Division of Water Quality to
continue the review of this project.
® Please supply confirmation from the US Army Corps of Engineers allowing you to permit
under a NW 12 and not a NW29.
Please submit this information or otherwise address this request within 30 calendar clays of the date of this
letter. If we do not receive this requested information within 30 calendar days of the date of this letter,your
project will be withdrawn and you will need to reapply with a new application and a new fee.
