Washington St. Greensboro, NC 27401 Subject: Hornaday Road Extension Project DWQ# 06-0138 Follow up inspection Guilford County Dear Mr. Partrick: On June 24, 2010, Amy Euliss and myself, Division of Water Quality (Division) regional office staff conducted a follow- up inspection of portions of the Hornaday Road Extension project with David Rudd, City of Greensboro staff, Marvin Leatherwood from Dane Construction, and Steve Saunders from Culp Brothers Inc. This inspection was conducted in order to address items identified as deficiencies during a previous inspection, as well as review the impacts to the Randleman Buffer as a result of construction of a bridge bent adjacent to a stream restoration area. During the inspection it was determined that the culvert installed in Stream 3 has been completed. Erosion control measures are present immediately above and below the culvert and should remain in place until the site is determined to be stable by the Division of Land Resources. At that time these areas must be brought into compliance with the plans approved by the Division during permitting. A final inspection of the project impacts will be conducted upon completion to ensure compliance with the permitted impacts. In addition, the inspection of the Bent adjacent to the stream restoration area, indicates that the pre-formed scour hole is no longer functioning properly. Stormwater is re-concentrating around the bridge bent and forming an erosion gully into the stream buffer. Randleman Buffer regulations require that all stormwater enter the protected buffer as sheet flow. The Division recently addressed this issue in a letter to Mr. Cordell on June 15, 2010. The City must review the location and effectiveness of a scour hole adjacent to the new bridge bent and propose a solution to the problem that currently exists. Please note that the scour hole is a condition of the permit issued to the NCDOT by the Division and therefore any modifications to the scour hole must be acceptable to NCDOT.
