Cobey, Jr., Secretary Director Mr. Jack Henderson 602 Brookside Camp Road Hendersonville, N.C. 28739 SUBJECT: Permit No. NCO071897 Authorization to Construct Henderson's Rest Home Henderson County Dear Mr. Henderson: A letter of request for an Authorization to Construct was received September 23, 1988, by the Division and final plans and specifications for the subject project have been reviewed and found to be satisfactory. Authorization is hereby granted for the construction of a 7,000 GPD wastewater treatment plant consisting of a bar screen, a 7,000 gallon aeration tank, a 1,535 gallon clarifier, a 1,400 gallon aerated sludge holding tank, dual 50 cfm blowers, dual 3.0 square foot tertiary filters, a hypochlorination unit, a 185 gallon chlorine contact tank, a 60 gallon post aeration tank, effluent weir and 360 linear feet of 2-inch gravity effluent discharge piping to serve Henderson's Rest Home with no discharge to the surface waters of the State. This Authorization to Construct is issued in accordance with Part III paragraph B of the NPDES Permit No. NC0071897 issued January 11, 1988, and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the conditions and limitations specified in Permit No. NC0071897. The Permittee must employ a certified wastewater operator in accordance with Part III paragraph C of the referenced permit. The sludge generated from these treatment facilities must be disposed of in accordance with G.S. 143-215.1 and in a manner approvable by the North Carolina Division of Environmental Management. The Asheville Regional Office, telephone number 704/251-6208 shall be notified at least twenty-four (24) hours in advance of operation of these facilities. Such notification to the Regional Supervisor shall be made during the normal office hour from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. Box 27687, Raleigh, North Carolina 27611-7687 Telephone 919-733 7015 An Equal Opportunity Affirmative Action Employer In event the facilities fail to perform satisfactorily in meeting its NPDES permit effluent limits,.the permittee shall take such immediate corrective action as may be required by this Division, including the construction of additional wastewater treatment and disposal facilities. Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been installed in accordance with the approved plans and specifications. Mail the Certification to the Permits and Engineering Unit, P.O. Box 27687, Raleigh, N.C. 27611-7687. A copy of the approved plans and specifications for the subject project must be retained by the applicant for the life of the project. The Permittee shall provide for the wastewater treatment plant the following items: a. a solid wooden fence b. reduced noise mufflers on blowers units C. a vegetative screen One (1) set of approved plans and specifications is being forwarded to you.
