On June 25, 2010, the Division of Water Quality (DWQ) received your application sets dated May 25,
2010 for the above referenced project. The DWQ has determined that your application was incomplete
and/or provided inaccurate information as discussed below. Please provide the following so that we may
continue to review your project.
Additional Information Requested:
Incorrect and/or missing wetland, intermittent-perennial stream and/or buffer impacts.
DWQ is unable to review this project without noted or corrected impacts reflected. Page 5 of
11, item 2f and 2h shows less than one acre of impact. DWQ will require a more specific
quantitative figure in that field. Item 3g shows 70 linear feet of stream impact; however the
tally reflected in 3g shows 50 linear feet of stream. Please supply one (1) original and four (4)
copies of the corrected figures.
2. Page I of 11 reflects Jennifer Locklear as the owner; however, Karen Wrigley signed on page
11 of 11. Ms. Wrigley's signature is not valid without authorization from Ms. Locklear.
Please supply one (1) original and four (4) copies of the signature page and an authorization
letter (if necessary).
Please submit this information within 30 calendar days of the date of this letter. If we do not receive this
requested information within 30 calendar days of the date of this letter, your project will be withdrawn and
you will need to reapply with a new application and a new fee.
This letter only addresses the application review and does not authorize any impacts to wetlands, waters or
