Local: None Or 10 04 Federal: USACE Nationwide General Permit 3 - Maintenance 91 Action ID Number SAW-2005-10677 State: General Water Quality Certification #3687 Local: None Project Description Replace existing 52'-5" long by 28' (clear roadway) wide concrete deck bridge with timber piles with a proposed 122.53' long by 33' (clear roadway) wide reinforced concrete bridge on steel piles. Age of the bridge and safety inspection reports dictate the need for replacement of this bridge. There will be no impacts to jurisdictional waters during bridge construction and guardrail installation. The longer bridge will result in approximately 3,201 square feet of excavated roadway fill beneath the new structure. This project does not involve Section 10 (Rivers and Harbors Act) navigable waters. This project does not require the completion of a formal PCN but notification to the Corps is being provided to be included in their database. This conveyance is shown on the Gardnerville USGS Quad Map and Sheet Number 69-A of the Soil Survey of Pitt County. Fork Swamp is classified as C;Sw;NSW and is in the Middle Neuse 03020202 RUC. The channel is 40 feet wide from bank top to bank top and drains a woodland/agricultural area. No federally protected species or historic resources are found immediately adjacent to this location. In order to ensure compliance with best management practices, all attached General and Regional Conditions associated with Nationwide Permit 3 and Water Quality Certification #3687 must be followed. Keep demolition and construction debris out of the stream. Riprap will not be placed below the plane of ordinary high water for stabilization. Excavation should be confined to the right of way, and limited to the minimum amount required to excavate in high ground for the construction of the new bridge. All excavated material should be removed from the site, placed on existing spoil piles, or disposed of in farm fields. Any waste material disposed of off-site shall have a reclamation plan. All Erosion and Sedimentation measures and Maintenance Best Management Practices should be implemented. I am forwarding a copy to Mr. Andy Blankenship in Roadside Environmental for his perusal in E&S inspections. The project supervisor should always have this permit package, including the attached General and Regional Conditions associated with the Nationwide Permit 3 and Water Quality Certification #3687 on site during the bridge replacement. Last Wednesday (February 23) 1 investigated the three project areas and determined that no properties listed on or eligible for the National Register of Historic Places are impacted at any of the permit sites. The projects are therefore in compliance with North Carolina General Statute 121-12(a) and Section 106 of the National Historic Preservation Act for historic architecture. They are: MA02017R (SR 1203, Allen Road); MA02008B (Bridge No. 37); and MA02014B (Bridge No. 23). By my reckoning one project in Greene County, MA02005R (NC 91), still needs investigation in the field. Four others may also require architectural survey - can you confirm the permit status for each of the following four projects? MA02024R (NC 58, Carteret); MA02021R (US 258, Jones); MA02020R (US 258, Lenoir); and MA02015R (NC 306, Pamlico). Please let me know of any changes, additions, or deletions. Thanks!
