and related information for the above referenced project. The DWQ has determined that the information
supplied was incomplete and/or provided inaccurate information as discussed below.
Additional Information Requested:
® Division of Water Quality requires five (5) complete sets to begin the review and approval
process. Please supply three (3) more sets.
Section A. 1. Processing was not completed in full. No information was supplied for Section
1 a, I b, 1 d and 1 e. It will be necessary to contact the USACE Regulatory Office in Asheville to
determine the Nationwide Permit number. Their telephone number is: 828-271-7980. Once
that has been determined, please complete page 1 of the PCN application and remit five (5)
copies.
Please submit this information within 30 calendar days of the date of this letter. This letter only addresses
the application review and does not authorize any impacts to wetlands, waters or protected buffers.
Please be aware that any impacts requested within your application are not authorized (at this time) by the
