On June 16, 2010 the Division of Water Quality (DWQ) received the revised PCN application sets for the
above referenced project. The DWQ has determined that your application was incomplete and/or
provided inaccurate information as discussed below.
Additional Information Requested:
® The original PCN reflected stream impact of 143 linear feet and the fee of $240 was included
with the project. The revised PCN reflects 165 linear feet of temporary impact and 65 linear
feet of permanent impact. This project will require an additional fee of $330. The total fee
required for an impact of 150 linear feet or more of stream is $570. A check will be needed
payable to Division of Water Quality to continue the review of this project.
Please submit this information or otherwise address this request within 30 calendar days of the date of this
letter. If we do not receive this requested information within 30 calendar days of the date of this letter, your
project will be withdrawn and you will need to reapply with a new application and a new fee.
This letter only addresses the application review and does not authorize any impacts to wetlands, waters or
protected buffers. Please be aware that any impacts requested within your application are not authorized (at
