highway drainage system at R 1978_(Hopson Road) address, in
Durham County, do hereby certify the following:

The Industrial 1 Commercial 1 Residential facility does not require an
NPDES stormwater permit.
E1 The Industrial f Commercial 1 Residential facility does require an NPDES
stormwater permit. The permit has been obtained and a Stormwater
Pollution Prevention Plan (SPPP) is in place. Appropriate structural
stormwater best management practices (BMPs) are designed and will be in
place as required by the North Carolina Department of Environment and
Natural Resources (NCDENR) audior the local governing agency. Ali
structural stormwater BMPs are located outside of NCDOT right-of-way.
I understand if the NCDOT determines the facility is not in compliance with
NPDES stormwater requirements, the Department will report the noncompliance
to the NCDENR Division of Energy, Mineral and Land Resources. I also
understand that falsification of this certification may result in penalty of law
against the facility and me as prescribed in the North Carolina General Statutes.
