Subject: Formal Consultation for the James spinymussel related to the North Carolina
Department of Transportation’s Bridge No. 8 replacement project

Dear Ms. Mizzi:

The North Carolina Department of Transportation (NCDOT) plans to submit a Pre-Construction
Notification for the Department of the Army authorization to replace a bridge (i.e., Bridge No. 8
on NC 704) that includes crossing the Dan River in Stokes County, North Carolina. Ms. Amy
Euliss, NCDOT, Division 9 Environmental Officer, is representing NCDOT in the processing of
this application.

I have reviewed the NCDOT’s biological assessment and concluded that the proposed work may
affect the James spinymussel (Parvaspina collina) and therefore, I am hereby requesting
initiation of formal consultation pursuant to Section 7 of the Endangered Species Act of 1973, as
amended.

Your expeditious review and preparation of the Biological Opinion will be appreciated. Should
