DANIEL SMITH Director Mr. Fredrick Hancock 3043 N NC Hwy 62 Burlington, NC 27217 SUBJECT: Dear Mr. Hancock: NORTH CAROL.INA Environmental Quality November 12, 2020 Compliance Evaluation Inspection Hancock Single Family Residence — Wastewater Irrigation System Permit No. WQ0001588 Alamance County On November 10, 2020, staff of the North Carolina Division of Water Resources Winston- Salem Regional Office (DWR) performed an announced routine compliance inspection of the subject wastewater irrigation system. This compliance inspection was conducted by DWR staff persons Justin Henderson and Alex Lowe. Review of the subject wastewater irrigation system reflects compliance with Permit No. WQ0001588. Please be aware in accordance with Permit Condition I. (1), a permit renewal shall be requested no later than six (6) months prior to the expiration of the subject permit, which will occur on July 31, 2021. Also, please remember per Permit Condition V. (2) that an inspection log for the wastewater system shall be maintained. This log shall include, at a minimum, the date and time of inspection, observations made, any maintenance, repairs, or corrective actions taken. This log shall be maintained for a minimum of five years. Residual removal events are considered as part of system maintenance, and as such recordation of these events shall be maintained as part of the subject inspection log. Please refer to the enclosed compliance inspection report form for additional observations and comments. Sand Filter/Treatment Pods *** Accessible sand filter surfaces shall be raked/leveled every 6 months and vegetative growth shall be removed manually. *** # Is system something other than a sand filter? # If YES, what kind?
