received June 11, 2020. However, additional information is required before the review may be completed.
Please address the items on the attached pages no later than the close of business on September 9, 2020.

rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Section A, or failure to provide the additional information on or before the above requested date may

1. DCAR Form 06-16 Lower Creek: Please submit the residual source certification (RSC) form for
Lower Creek. Please include the most recent data for nutrients, metals and TCLP. Also, fulfill the
requirements in conditions D) Program information in the DCAR form.
2. Form RSC 06-16 Town of Blowing Rock:
• The Sodium absorption ratio reported is incorrect. Please re -calculate and submit the
corrected value.
The TCLP data for Town of Blowing rock is incorrectly reported in mg/kg units. Please
re -submit the data in mg/L units and ensure that all the following parameters are listed in
the table 3) Hazardous waste determination of the RSC form:

3. Operation & Maintenance Plan: The updated Operations and Maintenance plan was previously
submitted on 11/5/2019 in response to an add -info. Please update the plan to include Town of
Blowing Rock and re -submit it pursuant to condition H) in the DCAR 06-16 form.
4. Fecal sludge results: Please verify if the fecal results submitted were for Lower Creek WWTP
source or Town of Boiling Rock. Fecal results are required for Town of Boiling rock to add it as a
source.
5. Vector reduction requirement for Town of Blowing Rock: Please submit the data pursuant to 15A
NCAC 02T .1107(a)(8) to ensure this requirement is being fulfilled.
