The Division received your request to rescind the subject NPDES wastewater permit. We do not
object to this request.
NPDES permit NCO020052 is hereby rescinded, effective immediately. This message is to close the
file for the Division, and for your records.
If in the future your utility wishes to discharge wastewater to the State's surface waters, you
must first apply for and receive a new NPDES permit.
If you have any questions concerning this matter, simply reply to this message.
PLEASE NOTE: as part of the Division's conversion to paperless communication, this email is the
only notification you will be sent regarding the permit rescission. No hard copy will be mailed.

I received DEQ's 11/8/19 letter waiving visitation and DMR submittal for this WWTP yesterday. DEQ's consideration of
the specific situation and approval of the waiver is much appreciated.
PERCS also approved the sewer project's final engineer certification on 11/21. A copy of the approval from the online
permit tracker is attached, as well as a link below to the tracker app. The sewer project permit # is WQ0038297.

Please also find attached the Town's August 16, 2019 request to rescind the permit. The City of Gastonia contract
operates McAdenville's WWTP and I am the technical point of contact. Our earlier understanding was that the NPDES
permit could not be rescinded until the PERCS unit approves the final engineer's certification for the sewer line project
that took the WWTP offline. If that is incorrect, a permit rescission at this time would be appreciated. Otherwise,
placing the permit in inactive status would also be appreciated so we can avoid the need to visit the site 5 days/week.
Please advise if additional information is needed.

Good Morning, wanted to share the attached photo of the completed plant demolition. The final engineer's certification
was submitted to the DEQ Collection System staff on 9/20. They indicate the reviews are backed up right now. As soon
as Collections has approved the sewer line project, I will send a copy to you. Until then, the W WTP site is being visited
5/week and no flow DMRs will be submitted. If more information is needed, please let me know.

Dear Ms. Scheringer:
As stated by 15A NCAC 02B.0506 (a) (1) (E), permittees must continue to submit monthly discharge
monitoring reports (DMRs) to the Division, even when there is no discharge from the facility during the
reporting period. The Director may waive this requirement during extended periods of no discharge.
I have reviewed your request dated October 10, 2019, which states that the subject facility has been
demolished. After reviewing your request and considering the recommendation of the staff of the
Mooresville Regional Office, I have agreed to waive the requirement to submit monthly DMRs for the subject
facility, effective November 1, 2019. The DMRs up through October 2019 should be submitted as usual.
Please be aware that should any discharge occur, the discharge must be monitored as per the terms of the
NPDES permit. Notify the Mooresville Regional Office of any pertinent developments that arise regarding
wastewater discharge from the site.
Please also be advised that while requirements for operator visitation, monitoring and reporting are being
waived during the period of no discharge, the permittee is still required to pay the Annual Administering
and Compliance Monitoring Fee that is associated with this and all NPDES permits.

Dear Permitting Unit Staff,
McAdenville's wastewater treatment plant has been taken offline by the Phase II South Fork Sewer
Project, which has rerouted the Town's wastewater flow into the City of Gastonia's collection system.
Sewer flow is no longer entering the treatment plant, sludges and solids have been removed from the
plant, and the piping has been disconnected such that the plant can no longer discharge to the South
Fork Catawba River. Demolition of the treatment plant structure is scheduled to be completed within
the next 90 days. Any rain water that accumulates in the treatment plant prior to demolition will be
pumped into the new collection system piping to ensure proper treatment. Additionally, ON Tuvia,
Environmental Engineer at the Mooresville Regional Office, completed a site visit on August 14, 2019.
A partial engineer's certification for the sewer project was submitted to and approved by PERCS Branch
staff, please see attached. The partial certification was necessary since flow would automatically be
routed into the new sewer infrastructure upon completion of three sewer line tie-ins. These three tie-
ins were completed after approval of the partial certification. The City of Gastonia's engineer is
compiling information now to submit the final engineer's certification. It is understood that the NPDES
permit cannot be rescinded until the final engineer's certification is approved by PERCS. The Town
respectfully requests permit rescission as soon as is feasible to avoid the expense of the required five
day per week visitation.

Dear Permitting Unit Staff,
Clean Water Management Trust Fund grant project number 2011-508 provides grant funds to install
sewer infrastructure that will enable the Town of McAdenville's wastewater treatment plant to be taken
offline. In accordance with the grant requirements, McAdenville hereby submits this letter as a
notification and request for rescission of the Town's NPDES permit once the sewer construction project
is completed.
Construction is scheduled to be completed in December 2018, upon which all wastewater currently
treated at McAdenville's WWTP will be discharged to the City of Gastonia's Two Rivers Utilities
collection system and ultimately be treated at the Long Creek WWTP (NPDES Permit # NC0020184).
When the project is completed, the Town will notify the Permitting Unit that the plant is no longer in
operation and certify that it has been decommissioned so that NCDEQ can conduct the site visit to
confirm decommissioning and proceed with permit rescission.

Graham's cooperation during the site visit was much appreciated. Please advise the staff involved
with this NPDES Permit by forwarding a copy of the enclosed report.
During the inspection the effluent pipe couldn't be reached. Right of way must be
maintained in order for the effluent pipe to be inspected.

Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant
to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between North Carolina and the U.S. Environmental Protection Agency dated October 15, 2007 (or as
subsequently amended).
This final permit contains no significant changes to its terms from those found in the draft permit sent to
you on October 22, 2014. However, you are also reminded of the new condition requiring electronic
reporting of discharge monitoring report (DMR) data using the Division's eDMR internet application.
This new requirement will become effective on November 1, 2015 (approximately 270 days or nine
months following the effective date of the permit).
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty
(30) days following receipt of this letter. This request must be in the form of a written petition,
conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of
Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless
such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Resources or
permits required by the Division of Land Resources, the Coastal Area Management Act or any other
Federal or Local governmental permit that may be required.

A. (2.) EFFLUENT MERCURY ANALYSIS
The Permittee shall provide one effluent mercury analysis, using EPA Method 1631 E, in conjunction
with the next permit renewal application. The analysis should be taken within 12 months prior to the
application date. Any additional effluent mercury measurements conducted from the effective date of
this permit and up to the application date shall also be submitted with the renewal application.
If the result of the mercury analysis is not provided with the application, the application may be
returned as incomplete and the Permittee considered non -compliant.

A. (3.) ELECTRONIC REPORTING OF DISCHARGE MONITORING REPORTS
Proposed federal regulations require electronic submittal of all discharge monitoring reports (DMRs)
and specify that, if a state does not establish a system to receive such submittals, then permittees must
submit DMRs electronically to the Environmental Protection Agency (EPA). The Division anticipates
that these regulations will be adopted and is beginning implementation in late 2013.

Beginning no later than November 1, 2015 (270 days.from the effective date of this permit), the
permittee shall begin reporting discharge monitoring data electronically using the NC DWR's
Electronic Discharge Monitoring Report (eDMR) internet application.
Monitoring results obtained during the previous month(s) shall be summarized for each month and
submitted electronically using eDMR. The eDMR system allows permitted facilities to enter
monitoring data and submit DMRs electronically using the internet. Until such time that the state's
eDMR application is compliant with EPA's Cross -Media Electronic Reporting Regulation
(CROMERR), permittees will be required to submit all discharge monitoring data to the state
electronically using eDMR and will be required to complete the eDMR submission by printing,
signing, and submitting one signed original and a copy of the computer printed eDMR to the

If a permittee is unable to use the eDMR system due to a demonstrated hardship or due to the
facility being physically located in an area where less than 10 percent of the households have
broadband access, then a temporary waiver from the NPDES electronic reporting requirements may
be granted and discharge monitoring data may be submitted on paper DMR forms (MR 1, 1. 1, 2, 3)
or alternative forms approved by the Director. Duplicate signed copies shall be submitted to the
mailing address above.
Requests for temporary waivers from the NPDES electronic reporting requirements must be
submitted in writing to the Division for written approval at least sixty (60) days prior to the date the
facility would be required under this permit to begin using eDMR. Temporary waivers shall be
valid for twelve (12) months and shall thereupon expire. At such time, DMRs shall be submitted
electronically to the Division, unless the permittee re -applies for and is granted a new temporary
waiver by the Division.

the issuance of the permit or in the case of a new facility, on the last day of the month following the
commencement of discharge.

All eDMRs submitted to the permit issuing authority shall be signed by a person described in Part
II, Section B. (11.)(a) or by a duly authorized representative of that person as described in Part II,

For eDMR submissions, the person signing and submitting the DMR must obtain an eDMR user
account and login credentials to access the eDMR system. For more information on North

Thank you for providing us the opportunity to comment on the draft renewal of the NPDES permit for
the McAdenville Wastewater Treatment Plant. We have reviewed this draft and wish to submit the
following request:
• We request that the deadline for submitting discharge monitoring reports through the eDMR
application be extended to 540 days from the effective date of this permit. We understand
and support DWR's efforts to shift to electronic reporting, however with the anticipated
closure of this facility in the spring of 2016, we feel there is little value in going through the
eDMR set-up process for this facility only to submit a handful of reports in the new format.
Provided the November 9, 2011 general conditions are included with the permit, we have no other

The Town requested that it receive an extension for the date by which it is to begin using
eDMR from 270 days to 540 days. The request was made in the hope that the extended
time would occur beyond the facility's scheduled connection to Two Rivers Utilities.
This request has been denied due to the uncertainty of construction schedules, the ease of

Form 2A has been developed in a modular format and consists of a "Basic Application Information" packet
and a "Supplemental Application Information" packet. The Basic Application Information packet is divided

1. All industrial users subject to Categorical Pretreatment Standards under 40 Code of Federal Regulations (CFR) 403.6 and
40 CFR Chapter I, Subchapter N (see instructions); and
2. Any other industrial user that:
a. Discharges an average of 25,000 gallons per day or more of process wastewater to the treatment works (with certain
exclusions); or
b. Contributes a process wastestream that makes up 5 percent or more of the average dry weather hydraulic or organic
capacity of the treatment plant; or
C. Is designated as an SIU by the control authority.

Sludge Management Plan

McAdenville Wastewater Treatment Plant
NPDES Permit # NCO020052
Sludge management at the McAdenville Wastewater Treatment Plant (WWTP) is handled as
follows:

• Influent grit settles in the grit chamber prior to the flow entering the treatment plant.
• Biological solids are settled in the plant's clarifier section and pumped to its aerobic
digester section where it is aerated.
• Settled solids from both sources are pumped out by vacuum truck and transported to the
influent of one of Two Rivers Utilities' treatment facilities (either Long Creek W WTP:
NPDES Permit #NC0020184, Crowders Creek WWTP: NPDES Permit #NC0074268, or
Eagle Road WWTP: NPDES Permit #NC0006033). This wastewater is monitored as part
,. of the influent flow for these plants.
• The solids undergo the full treatment process at that facility and are disposed of in
accordance to their respective permits. Biosolids from these facilities are land applied
.. under Land Application of Residual Solids (503) Permit 4W00001793 and South
Carolina Land Application of Domestic Wastewater Sludge Permit #ND0084883.
