Directions to site: From the intersection of I-85 and Lane Street in Kannapolis (exit 63), travel east on lane street approximately 2.0 miles to the intersection with Wright Avenue. Turn right on Wright Avenue and travel approximately 50 feet to the intersection with Central Avenue. The site is located at the corner of Central Avenue and Wright Avenue. 6. Discharge point(s), List for all disposal sites: Latitude: 35' 30' 10" Longitude. 80' 35' 30" USGS Quad No.: E 16 SE U.S.G.S. Name: China Grove, N.C. 7. Size (land available for expansion and upgrading): Limited area is available for expansion and upgrading. 8. Topography (location map or topographic indicating relationship to 100 year flood plain included): Sloping at the rate of 1%. The site is not located in a flood plain area. 9. Location of nearest dwelling and water supply well: The nearest dwelling and well are approximately 350 feet from the site. 10. Watershed stream basin information: Unnamed tributary to Concord Lake a. Watershed Classification: WS IV b. River Basin and Subbasin No.: 030712 C. Distance to surface water from disposal system: Approximately 2000 feet. PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS 1, a. Volume of wastewater to be permitted: 10 GPM (Design Capacity) b. Prevalent toxic constituents in wastewater: Benzene, Toluene, Ethylbenzene, xylenes, Methyl-t-Butyl Ether (MTBE), IPE and Naphthalene. f. Sludge handling and disposal scheme: According to applicant, spent granular activated carbon will be properly manifested and handled as per North Carolina State Guidelines. 6. Is this facility being constructed with Construction Grant Funds (municipals only)? N/A 2. Special monitoring requests: Toxicity may need to be limited/monitored due to the nature of such discharges. Important SOC, JOC or compliance Schedule dated: (please indicate)? N/A Alternative Analysis Evaluation: Has the facility evaluated all of the non -discharge options available. Please provide regional perspective for each option evaluated. Spray irrigation: This option has been evaluated and found not to be an alternative due to limited space. Connection to regional Sewer System: The discharge to the Cabarrus County Sewer Authority is very costly. The yearly administrative and permitting fees for discharge into the sanitary sewer would be approximately $160,000 to $175,000 yearly for a 10gpm discharge according to the applicant. Subsurface disposal: This option has been evaluated and found not to be an alternative due to high water table elevation. 5. Air quality and/or groundwater concerns or hazardous materials utilized at this facility that may impact water quality, air quality, or groundwater? Contaminated groundwater already exists at the site. An air quality permit for this facility may be required. 6. Other Special Items: Discharge is to an unnamed tributary to Concord Lake and not to Fisher Lake as mentioned in the report. PART IV - EVALUATION AND RECOMNIENDATIONS Crown Central Petroleum, is requesting a discharge permit for the treatment of contaminated groundwater at the former Fast Fare store located at 701 Wright Avenue in Kannapolis at the Cabarrus/Rowan County line. The contaminated groundwater was allegedly caused by a leaking underground gasoline tank. The proposed interim remediation is to minimize the migration of the contaminated groundwater to potable wells in the area. Since the discharge is proposed to be to a roadside ditch, it is recommended that the applicant obtain approval from the Department of Transportation and/or the City of Kannapolis. It is recommended that a discharge permit be issued pending review and approval by P & E and the Groundwater Section. This NOI has been assigned the application number shown above. After a review of the permit application it was found to be incomplete. Before I can issue the permit I will need the following information. Additional Information Required; • Monitoring data less than a year old needs to be submitted with the application. Perform a present value of costs analysis for both the proposed system discharging as described in this application and discharging into the sanitary sewer and submit the results. I have enclosed some guidance material, found specifically in Appendix A, to help with this analysis. Please be aware that the Division's regional office, copied below, must provide recommendations from the Regional Supervisor prior to final action by the Division. Spray irrigation The alternatives to discharge analysis should include boring logs and/or other information indicating that a subsurface system is neither feasible nor practical as well as written confirmation indicating that connection to a POTW is not an option. It should also include a present value of costs analysis as outlined in the Division's "Guidance For the Evaluation of Wastewater Disposal Attematives". 11) Additional Application Requirements: For new or proposed discharges, the following information must be included in triplicate with this application or it will be returned as incomplete. a) 7.5 minute series USGS topographic map (or a photocopied portion thereof) with discharge location clearly indicated. b) Site map, if the discharge is not directly to a stream, the pathway to the receiving stream must be clearly indicated. This includes tracing the pathway of a storm sewer to its discharge point. c) If this application is being submitted by a consulting engineer (or engineering firm), include documentation from the applicant showing that the engineer (Or firm) submitting the application has been designated an authorized Representative of the applicant. d) Final plans for the treatment system (if applicable)_ The plans must be signed and sealed by a North Carolina registered Professional Engineer and stamped -"Final Design -Not released for construction". e) Final specifications for all major treatment components (if applicable). The specifications must be signed and sealed by a North Carolina registered Professional Engineer and shall include a narrative description of the treatment system to be constructed. f) Site map identifying the location of the monitoring wells, the recovery wells, and the treatment system unit. g) Analytical monitoring data which sufficiently characterizes the type and concentration of contaminants on site. This"includes a listing of any chemicals found with the maximum observed concentrations reported. The data provided must be no older than one year prior to the date of this application. At a minimum, the following parameters must be reported: Benzene, Toluene, Ethylbenzene, Xylene, Lead, Naphthalene, Phenol, IVITBE, 1,2-Dichlorethane, Fluorene and Phenanthrene.
