atAie-subj-en October-16, 20-1-5Lby Mr—Ori-T-uvia Y-our-coaper-ation during tl sits -
visit was much appreciated. Please advise staff involved with this NPDES Permit of the results
of this inspection by forwarding a copy of the enclosed report.
As was discussed with your onsite representative Mr. Scott Allen, effluent flow should be
monitored bi-annually, and recorded on the discharge monitoring report (DMR). If no flow is
present, "No Flow" should be recorded on the DMR. Also please note that all permit records
should be kept for at least three years.
The attached report, should be self-explanatory; however, should you have any questions,
please do not hesitate to contact Mr. Tuvia at (704) 235-2190, or via email at ori.tuvia@ncdenr.gov.

f. Possible toxic impacts to surface waters: There are no toxic impacts expected from the
operation of this treatment system. If coagulant additives are necessary to effect
compliance, then approval of these products will be required prior to their use.
2. Treatment plant classification: Class I (proposed).
3. Compliance Background: Proposed facility, no compliance history at this location.
PART III - OTHER PERTINENT INFORMATION
Special monitoring or limitations (including toxicity) requests: The applicant has applied for
coverage under the General Permit for sand dredging.
PART IV - EVALUATION AND RECOMMENDATIONS
Carolina Sand, Inc. (CSI) has applied for coverage under the General Permit for sand dredging
for a site adjacent to Grants Creek near the confluence with the Yadkin River. CS plans to mine
accumulated sand both above and below where Grants Creek enters the Yadkin and market this sand
for sale. The proposed CS wastewater treatment facility would be constructed on a piece of property
which has been leased from the City of Salisbury and is partially occupied by the City's Grants Creek
WWTP.
CS has proposed to discharge up to 1.0 MGD of treated water from proposed settling basins
into a UT to Grants Creek. At the time of the initial site investigation (March 2nd) and on a follow-up
visit (March 1 lth), this UT was observed to have flow due to recent heavy rainfall events. Stream
characteristics, however, are more typical of an ephemeral (and possibly) intermittent chantiel, with
flow predominately driven by rainfall events. Given this scenario, MRO staff has concern with
regards to the release of up to 1.0 MGD of wastewater into a channel that may not be sufficient to
handle this quantity of water. Field observations note the potential for scouring of the channel,
resulting in sedimentation loss and down stream channel degradation.
Immediately below the proposed point of discharge, the channel empties into a floodplam area
that captures overflow water from Grants Creek and the nearby Yadkin River during heavy rain events.
There was an extensive amount of water standing in the flood plain during the March 1 lth visit and
this was documented with photos that have been forwarded to the CO. Any sedimentation loss, either
from impacts to the receiving channel or from the proposed treatment work will likely settle into this
wetland area resulting in unauthorized fill and possible wetland degradation.
Page Three
Such being the case, this Office recommends that the point of discharge be relocated to a
location where impacts from the release of this amount of water are less likely. Possible discharge
locations that should be considered are to the main stem of Grants Creek or the Yadkin River. Either
of these suggested locations would avoid the potential impact of releasing a large quantity of water into
a channel that is not sufficiently sized to distribute the water to down stream sources. These
alternative discharge locations will also avoid potential wetland impacts from sedimentation loss and
deposition. To access either Grants Creek or the Yadkin River, CSI may have to obtain approval from
the property owner who owns the land between the CSI site and the river (Alcoa), and/or the FERC
who regulates that portion of the River (also considered the uppermost portion of High Rock Lake).
Pending a satisfactory resolution to this matter, it is recommended that a General Permit be
issued for the proposed discharge.

This letter is in response to your April 06, 2011 site visit regarding the proposed discharge of effluent
water into the UT to Grants Creek.
Please see the attached Mine Map for information on the proposed discharge system. Per
recommendations from NCDWQ the plans have been revised to show that effluent water piped from
settling basin two will now be discharged back into the Yadkin River (WS — V). The proposed discharge
pipe will be laid upon the existing ground surface and cause no jurisdictional impacts. A booster pump
will be used to supplement when required.

RE: Grants Creek Mine Site
Mr. Chapin,
The following revisions have been made to the Grants Creek Mine Site plans. The stockpile, junction structure and
proposed impact of 149' of 24" CMP has been removed from the plans. A 50' buffer has been provided to ensure no
jurisdictional impact. Further we have included data sheets and provided notes on the plans/maps that are attached
that indicate that the basins and stockpile area will be installed on high ground and no jurisdictional impacts will occur.
Please accept this letter as a request to withdraw our application for a nationwide 39, but please still consider our
request for the nationwide 16 valid.
In addition we have added a level spreader device to diffuse our discharge into the UT to Grants Creek. (no Jurisdictional
Impact). For the convenience of NCDWQ of have copied Mike Parker with this packet. Thanks for your assistance and
please contact me at as you need to.
Thanks,
Derek Goddar

Subject: Renewal Notice / General Permit NCG520000
Certificate of Coverage NCG520009

Dear Permittee:
You are receiving this notice because your company currently operates a dredging operation covered under the
subject General Permit for the discharge of sand -dredging runoff. NCG520000 will expire on July 31, 2007. Federal (40 CFR
122.41) and North Carolina (15A NCAC 2H.0105(e)) regulations require that permit renewal applications be filed at least 180
days prior to expiration of the current permit. To satisfy this requirement, the Division must receive a renewal request
postmarked no later than February 1, 2007.
The Certificate of Coverage (CoC) specific to your operation was last issued on August 1, 2002. The Division needs
information from you to determine if coverage under NCG520000 is still necessary.
➢ If your property still has a wastewater system like the ones described in the enclosed Technical Bulletin, you must renew the
subject CoC. Complete the enclosed form and submit it to the address on the form.
➢ If you are not sure what type of system your property has, contact Wes Bell in the NC DENR Mooresville Regional Office at
(704) 663-1699. That person [or other staff members] can help you determine if you should renew your CoC.
➢ If this site has been shut down and no further dredging will occur, contact me at the address or phone number listed below to
request rescission of the CoC.
Please verify that the provided information is correct, or make corrections on the attached form. Complete the
additional questions, then sign and date the form. The completed form should be submitted to the address listed below the
signature block.
If you have any questions concerning this matter, please contact me at the telephone number or e-mail address listed
below. r t
Sincerely,
