Good Afternoon,
In accordance with the General Permit NCG03000 Tier Three requirements, we are hereby notifying the DEMLR Regional
Engineer that we have exceeded the benchmark value on four occasions. Per the Tier Level requirements we have
completed the following:
• Conducted a stormwater management inspection of the facility
• Identified an evaluated possible root causes of the benchmark value exceedance
Identified potential and feasible controls and physical improvements and implemented corrective actions
consisting of:
o Filled in washed out area and covered it with ground netting; installed rock over netting to prevent
future erosion and installed two nets across the ditch with grass rolls
o Changed out filters on torc fan blade saw
o Practice good housekeeping when changing filters on torc fan blade saw
o Redesigned cover over drum to enclose it for dust from torc fan blade saw
o Good housekeeping measures - cleaned up all debris in scrap yard and between buildings to eliminate
run off into drains
• Implemented monthly analytical sampling and qualitative monitoring for all parameters of the outfalls
• All items have been recorded in the SPPP

this time. We are still evaluating if there are any additional root causes while we await the October monitoring results.
