Per your request, the semi-annual effluent flow estimates are detailed below. For the reporting period beginning September 1, 2016 and ending December 31, 2016 the greatest amount estimated effluent flow was 20 cubic yards. This was arrived at by calculating the following: The great amount removed during one day during the reporting period occurred on September 29, 2016. This information was reported to the City by Clark Ledbetter, who holds the dredging contract. The totals for that measured 100 cubic yards (8 loads). The formula you provided is applied below. 100 cubic yards * 0.20 = 20 cubic yards If you have questions, or need additional information, please do not hesitate to contact the Planning office. The plans and specifications submitted were prepared under the supervision of Mr. Christopher E. Wood, PE. This letter constitutes approval of the proposal to install instrumentation within the subject dam according to the plans and specifications received by this Division on August 28, 2015, with the following stipulations: 1. Project construction shall be supervised by Mr. Christopher E. Wood, PE, Mr. Christopher E. Wood, PE, shall be responsible for field observation of construction as necessary to ensure compliance with approved plans [GS 143-215.29]. 2. During installation, the Division of Energy, Mineral, and Land Resources may require such progress reports as are deemed necessary. 3. In accordance with GS 143-215.29 and NCAC 15A-2K .0203, .0212, .0215, and .0216, within 30 days of completion of the project, Mr. Christopher E. A procedure and schedule for reading the monitoring wells, recording the data, and storing the data shall be provided with the record drawings. Until the ongoing investigation and repair work is complete, monitoring of the piezometers shall be performed on at least a daily basis while the low level conduit remains unpressurized and hourly monitoring shall be performed at times when the low level conduit is pressurized. Piezometer readings shall be recorded and made immediately available upon request of this Division. A summary of piezometer readings shall be made in the weekly status reports required by Stipulation 5 of our letter to you dated July 20, 2015. 5. This approval does not convey the right to access the private property of others. Any required access to perform the approved work must be secured prior to initiation of construction activities. 6. The City owns the lake and for years has dealt with accumulations of sand and silt that continues to be deposited in the upper reaches of the lake. Past attempts to remove sand deposition in the main body of the lake have met with varying degrees of success, so the City is looking at trying to capture as much of the sand and silt as possible before it enters the lake. The City plans to use along -reach trackhoe to gain access to the material as it is carried down the creek. The material will be pulled from the creek and allowed to drain into the settling basin where the ancillary water will be treated and discharged. Once the material has dried,.it will be removed from the site. The proposed site was once used as a sand dredging operation in the past; however, the previous operation did not have a point source discharge and no discharge permit was required. The current site has limited area, which may become problematic for the City should the amount of deposition recovered exceed the amount of storage area available. It will also be important to maintain the existing basin from accumulating excess material which would reduce settling efficiency, especially since it is not baffled and is limited to a single basin. Water draining from the sand piles and stormwater runoff is collected in the sediment basin. Existing treatment facilities should be described in detail. Design criteria and/ operational data (including calculations) should be provided to ensure that the facility can comply with the requirements of the General Permit: The treatment shall be sufficient to meet with the limits set by the general permits. The trapping efficiency should be greater than 75%. The surface area should be as large as possible to insure sedimentation occurs. To secure optimum efficiency the flow length of the basin to the basin width should have a ratio of 2:1 Note: - Construction of any wastewater treatment facilities requires submission of three (3) sets of plans and specifications along with the application. Design of treatment facilities must comply with requirements of 15A NCAC 2H .0138. If construction applies to this discharge, include the three sets of plans and specifications with this application. It should also include a present value of costs analysis as outlined in the Division's "Guidance For the Evaluation of Wastewater Disposal Alternatives". 10) Additional Application Requirements: For new or proposed discharges, the following information must be included in triplicate with this application or it will be returned as incomplete. a) 7.5 minute series USGS topographic map (or a photocopied portion thereof) with discharge location clearly indicated. b)-.-Site-map; if the discharge is not directly to, a stream, the pathway to the receiving stream must be clearly indicated. This includes tracing the pathway of a storm sewer to its discharge point. c) If this application is being submitted by a consulting engineer (or engineering firm), include documentation from the applicant showing that the engineer (Or firm) submitting the application has been designated an authorized Representative of the applicant. d) Final plans for the treatment system (if applicable). The plans must be signed and sealed by a North Carolina registered Professional Engineer and stamped -"Final Design -Not released for construction". e) Final specifications for all major treatment components (if applicable). The specifications must be signed and sealed by a North Carolina registered Professional Engineer and shall include a narrative description of the treatment system to be constructed.
