Pierce & Associates Pinewood Manor Rest Home Spray Irrigation System Hertford County Dear Mr. Pierce: In accordance with your application received February 11, 1987, we are forwarding herewith Permit No. 6115-R3, dated January 29, 1988, to A. S. Pierce and Associates for the continued operation of the subject non -discharge type waste treatment facilities. This permit shall be effective from the date of issuance until January 31, 1989, and shn11 be subject to the conditions and limitations as specified therein. If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150E of North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer 11666, Raleigh, North Carolina 27604. Unless such demands are made, this permit shall he final and binding. ENVIRONMENTAL MANAGEMENT COMMISSION DEPARTMENT OF NATURAL RESOURCES AND.COMMUNITY DEVELOPMENT RALEIGH P E R M I T For the discharge of Sewage, Industrial Wastes, or Other Wastes In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Rules, and Regulations PERMISSION IS HEREBY GRANTED TO A. S. Pierce and Associates Hertford County FOR THE continued operation of a 7,500 GPD spray irrigation wastewater system consisting of • an influent pump station with dual 20 GPM grinder pumps, a 500,000 gallon stabilization/holding lagoon, a 115 GPM spray pump, a chlorine injection system and approximately 3.6 acres of irrigation area to serve Pinewood Manor Rest Home with no discharge to the surface waters of the State, pursuant to the application received February 11, 1987, and in conformity with the project plan, specifications, and other supporting subsequently filed and approved by the Department of Natural Resources and Community Development and considered a part of this permit. This permit shall be effective from the date of issuance until January 31, 1989, and shall be subject to the following specified conditions and limitations, and shall void Permit No. 6115R2 dated November 4, 1986. 1. This permit shall become voidable unless the facilities are constructed in accordance with the approved plans, specifications and other supporting data. 2. This permit is effective only with respect to the nature and volume of wastes ' described in the application and other supporting data. c3. The facilities shall be properly maintained and operated at all times. 14. This permit is not transferable. 5. This permit shall become voidable in the event of failure of the soil to adequately absorb the wastes and may be rescinded unless the facilities are installed, maintained, and operated in a manner which will protect the assigned . water quality standards of the surface waters and groundwaters. i6. In event the faci_Iities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take such immediate corrective action as may be required by this Division, including the construction of additional or replacement wastewater treatment or disposal facilities. 7. Any monitoring deemed necessary by the Division.of Environmental Management to insure surface and groundwater protection will be established and an acceptable sampling reporting schedule shall be followed. 8. The sludge generated form these treatment facilities must be disposed of in z accordance with General Statute 143.215.1 and in a manner approvable by the North Carolina Division of Environmental Management. 9...A suitable year round vegetative cover shall be maintained. 10.E Irrigation shall not be performed during inclement weather or when the ground is in a condition that will cause runoff. ll..Dikes shall be constructed so as to prevent surface water runoff from the spray field. 12.. Diversion or bypassing of the untreated wastewater from the treatment facility is prohibited. 13. The Permittee, at least six (6)-months prior to the expiration of this permit, • shall request its extension. Upon receipt of the request, the Commission will k review the adequacy of the facilities described therein, and as indicated, will extend the Permit for such period of time and under such conditions and limitations as it may deem appropriate. 14. The Permittee shall employ a certified wastewater treatment plant operator in ,responsible charge of the wastewater treatment facilities. Such operator most hold a certificate of the grade at least equivnlent.to the classification assigned to the wastewater treatment facilities by the Certification Commission. ,/ 15. Freeboard in the holding lagoon shall not be less than one (1) at any time. ✓ 16. The application rates shall not exceed .6 inches/acre/week. ,/17. The minimum buffer separation from the edge of the wetted area and the Rest Nome shall be 200 feet. /18. No wastewater shall be sprayed within 75 feet of'the adjacent property owner's land. 19. Within 90 days of permit issuance, three monitor wells, one upgradient and two downgradient, should be installed to monitor groundwater quality. The location and construction details for these wells should be by approval of the Washington Regional Office, from which a well construction permit must be obtained. 0 20. The monitor wells should be sampled initially after construction and thereafter every March, July, and November for the following parameters. NO3 pH Total Ammonia TOC TDS Chloride Water Levels The measurement of water level should be made prior to sampling for the remaining parameters. 21. The following monitoring schedule for the treatment system shall be submitted monthly to Mr. Jim Mulligan, Washington Regional Supervisor, Division of Environmental Management, P. 0. Box 1507 Washington, N. C. 27889, no later than the end of the following month. The soils and application rates shall be re-evaluated by a recognized soil scientist and a copy of his recommendations submitted to the Washington Regional Office at least six months prior to the permit expiration,date. 23. A minimum chlorine residual of 0.5 PPM shall be maintained at the distribution system and shall be monitored weekly at the fartherest point in the system.
