In accordance with your letter of request received January 2, 1986, we are
forwarding herewith Permit No. 6115-112, dated November 4, 1986, to Pinewood Manor Rest
Home, Inc. Mr. Lawrence Myers, Owner for the continued operation of the subject
non -discharge type waste treatment facilities.
This permit shall be effective from the date of issuance until November 30,
1991, and shall be subject to the conditions and limitations as specified therein, and
shall void permit No. 6115R dated, March 31, 1983.
If any parts, requirements, or limitations contained in this permit are
unacceptable to you, you have the right to request an adjudicatory hearing upon
written request within 30 days following receipt of this permit. This request must be
in the,form of a written petition, conforming to Chapter 150E of North Carolina
General Statutes, and filed with the Office of Administrative Hearings, Post Office
Drawer 11666, Raleigh North Carolina 27604.
If you wish to have the hearing before the Hearing Officer with this
Department, you must indicate in the petition that you waive the right to have the
contested case conducted by a Hearing Officer in the office of Administrative
Hearings, and wish to have the matter conducted in the Department of Natural Resources
and Community Development. Unless such demands are made this permit shall be final
and binding.

For the discharge of Sewage, Industrial Wastes, or Other Wastes
In accordance with the provisions of Article 21 of Chapter 143, General
Statutes of North Carolina as amended, and other applicable Laws, Regulations
PERMISSION IS HEREBY GRANTED TO

FOR THE
continued operation of a 7,500 GPD spray irrigation wastewater system consisting of
an influent pump station with dual 20 GPM grinder pumps, a 500,000 gallon
• stabilization/holding lagoon, a 115 GPM spray pump, a chlorine injection system,
approximately 3.6 acres of irrigation area, and associated valves, sprinklers and
pipes to serve the Pinewood Manor Rest Home; with no discharge of wastewater to the
surface waters of the State,
pursuant to the letter of request received January 2, 1986, and in conformity -with
the project plan, specifications, and other supporting data subsequently filed and
approved by the Department of Natural Resources and Community Development and
considered a part of this permit.
This permit shall be effective from the date of issuance until November 30,
1991, and shall be subject to the following specified conditions and limitations and
shall void Permit No. 6115R, dated March 31, 1983.
1. This permit shall become voidable unless the facilities are constructed in
accordance with the approved plans, specifications and other supporting data.
2. This permit is effective only with respect to the nature and volume of waters
described in the application and other supporting data.
3. The facilities shall be properly maintained and operated at all times.
4. This permit is not transferable.

5. The facilities shall be effectively maintained and operated at all
times so that there is no discharge to the surface waters, nor any contamination
of ground waters which will render them unsatisfactory for normal use. In event
the facilities fail to perform satisfactorily, including the creation of nuisance
conditions, or failure of the irrigation area to adequately absorb the
wastewater, the Permittee, shall take such immediate corrective action as may be
required by the Division of Environmental Management.
6. In event the facilities fail to perform satisfactorily, the Permittee shall take
such immediate corrective action as may be required by this Department.
7. Solids, sludges, or other pollutants removed or resulting from the wastewater
storage facilities shall be contained and disposed of in such a manner as to
prevent any contamination of the surface or groundwater of the State.
8. The Permittee shall employ a certified wastewater treatment plant operator
in responsible charge of the wastewater treatment facilities. Such operator must
hold a certificate of the grade at least equivalent to the classification
assigned to the wastewater treatment facilities by the Certification Commission.
9. Diversion or bypassing of the untreated wastewater from the spray irrigation
facility is prohibited.
10. A suitable year round vegetative cover shall be maintained and harvested on a
regular basis.
11. Irrigation shall not be performed during inclement weather or when the ground is
in a condition that will cause runoff.
12. Dikes shall be constructed so as to prevent surface water runoff from the spray
field.
13. The Permittee, at least six (6) months prior to the expiration of this Permit,
shall request its extension. Upon receipt of the request, the Commission will
review the adequacy of the facilities described therein, and in indicated, will
extend the Permit for such period of time and under such conditions and
limitations as it may deem appropriate.
14. Freeboard in the stabilization/holding lagoon shall not be less than one (1) foot
at any time.
15. The application rate shall not exceed 0.6 inches/acre/week.
16. The minimum buffer separation from the edge of the wetted area and the hest Home
shall be 200 feet.
17. No wastewater shall be sprayed within 75 feet of the adjacent property owner's
land.

18. Within 90 days of permit issuance, three monitor wells, one upgradient and two
downgradient, should be installed to monitor groundwater quality. The location
. and construction details for these wells should be by approval of the Washington
Regional Office, from which a.well construction permit must be obtained.
. 19. The monitor wells should be sampled initially after construction and thereafter
every March, July and November for the following parameters:

The measurement of water level should be made prior to sampling for the remaining
parameters.
20. The following monitoring schedule for the treatment system shall be submitted
monthly to Mr. Jim Mulligan, Washington Regional Supervisor, Division of
Environmental Management, P. 0. Box 1507, Washington, N.C. 27889, no later than
the end of the following month.

* A minimum chlorine residual of 0.5 PPM shall be maintained at the distribution
system and shall be monitored at the fartherest point in the system.
