REQUEST FOR MORE INFORMATION
Dear Mr. Wester:
On March 15, 2007, the Division of Water Quality (DWQ) received your application dated March 15,
2007 to impact 0.487 acres of forested-herbaceous wetlands to construct the proposed park. The DWQ
has determined that your application was incomplete and/or provided inaccurate information as discussed
below. The DWQ will require additional information in order to process your application to impact
protected wetland, streams and/or buffers on the subject property. Therefore, unless we receive the
additional information requested below, we will have to move toward denial of your application as
required by 15A NCAC 2H .0506 and 2B .0259(8) and will place this project on hold as incomplete until
we receive this additional information. Please provide the following information so that we may continue
to review your project.
Additional Information Requested:
1. Diffuse Flow
In the Tar Pamlico River Basin, you must show that your site is providing diffuse flow as required
by [15A NCAC 02B .0259(5)]. Please show your diffuse flow measures on your plan sheet,.and
provide all applicable worksheets, and a signed and notarized operations and maintenance
agreement.
2. Preconstruction Notification Form (PCN)
You have submitted an outdated version of the PCN form. Please resubmit your application
information on the March 2005 version. This version can be downloaded from the following
website: htt~//h2o enr.state.nc.us/ncwetlands/re~;cert.html.
Please contact the DWQ within three weeks of the date of this letter to verify that you have received this
letter and to provide us with a date that we may expect to receive this requested information. If we do not
receive this requested information within one week of the date you indicate it will be forthcoming, your
project will be withdrawn and you will need to reapply with a new application and a new fee. Please o°e

you within three weeks, we will assume that you no longer want to pursue this project and we will
consider the project as withdrawn.
This letter only addresses the application review and does not authorize any impacts to wetlands, waters
or protected buffers. Please be aware that any impacts requested within your application are not
