Dear Mr. Stephenson,
In accordance with the application received on July 20, 2010 and submitted on your
behalf by Diana Lanier, ATC Associates Inc., we are forwarding herewith Well Construction
Permits numbered WM0100237, WM0100238, and WM0100239 dated July 22, 2010 issued for
the construction of five monitoring wells as described in the applications and attached permits.
These monitoring wells will be drilled on property owned by Chuck N. and Mattie Triplett
(WM0100237), Walter Moore (WM0100238), and Cajah's Mountain Park, LLC (WM0100239).
These permits will be effective from the date of their issuance and shall be subject to the
conditions and limitations as specified therein. Well Construction Records (GW-la) are to be
Completed and signed by the certified drilling contractor for each well. These form(s) should be
submitted to the Department upon completion of the well(s). Please note attached permit
conditions.

In accordance with the provisions of Article 7, Chapter 87, North Carolina General Statutes, and
other applicable Laws, Rules and Regulations,
PERMISSION 15 HEREBY GRANTED TO
Cary Oil Company
FOR THE CONSTRUCTION OF one (1) monitoring well in the Inner Piedmont Geologic Belt, located
on property owned.by Chuck N. and Mattie Triplett in accordance with the application dated July 22,
2010, and in conformity with specifications and supporting data, all of which are filed with the
Department of Environment and Natural Resources and are considered a part of this Permit.
This Permit is for well construction only, and does not waive any provisions or requirements of
any other applicable laws or regulations.
Construction of wells under this Permit shall be in compliance with the North Carolina Well
Construction Regulations and Standards, and any other State and local laws and regulations pertaining
to well construction.
This Permit will be effective for the construction of monitoring wells from the date of its issuance
and shall be subject to other specified conditions, limitations, or exceptions as follows:
1. You are required to provide a copy of this permit to the property owners;
2. You are required to provide notification to the property owner upon well completion and upon
termination of monitoring; and,
3. The monitoring well shall be maintained in accordance with 15A NCAC 2C .0108.
