Dear Mr. Rudisill,
In accordance with the application received on August 23, 2010, and additional
information received September 3, 2010, and submitted on your behalf by ARM Environmental
Services Inc., we are forwarding herewith Well Construction Permit numbered WM0100240
dated September 03, 2010 issued for the construction of two monitoring wells as described in
the applications and attached permit. These monitoring wells will be drilled on property owned
by Caldwell County (PIN#27876000096).
This permit will be effective from the date of issuance and shall be subject to the
conditions and limitations as specified therein. A separate well Construction Record (GW-1a) is
to be completed and signed by the certified drilling contractor for each well. These forms
should be submitted to the Department upon completion of the wells. Please note attached
permit conditions.

PERMIT FOR THE
CONSTRUCTION OF A WELL
In accordance with the provisions of Article 7, Chapter 87, North Carolina General Statutes, and
other applicable Laws, Rules and Regulations,
PERMISSION IS HEREBY GRANTED TO
USGS
FOR THE CONSTRUCTION OF Two(2) monitoring wells in the Inner Piedmont Geologic Belt, located
on property owned by Caldwell County (PI1\1#27876000096) in accordance with the application dated
August 20, 2010, and in conformity with specifications and supporting data, all of which are filed with the
Department of Environment and Natural Resources and are considered a part of this Permit.
This Permit is for well construction only, and does not waive any provisions or requirements of
any other applicable laws or regulations.
Construction of wells under this Permit shall be in compliance with the North Carolina Well
Construction Regulations and Standards, and any other State and local laws and regulations pertaining
to well construction.
This Permit will be effective for the construction of monitoring wells from the date of its issuance
and shall be subject to other specified conditions, limitations, or exceptions as follows:
1. You are required to provide a copy of this permit to the property owners;
2. You are required to provide notification to the property owner upon well completion and upon
termination of monitoring; and,
3. The monitoring wells shall be maintained in accordance with 15A NCAC 2C .0108.
4. The monitoring wells shall be no closer than 25 feet from any property boundary.
