Page 11 of 11 PCN Form - Version 1.3 December 10, 2008 Version March 8, 2010 The Neighborhoods of Holly Ridge Wetland Stabilization Narrative Introduction We are proposing remedial work in and around waters of the U.S. in response to concerns noted in a February 24, 2010 letter from the USACOE. This work will include the completion and stabilization of temporary impacts approved in Action ID: SAW-2006-32163 (expired 9/18/2009) and the retrofit of a crumbling drainage structure. This drainage structure is creating a sedimentation problem in an adjacent stream and posing a hazard to the public due to its condition and proximity to residences. This work will require a Department of the Army verification to utilize a Nationwide Permit. Impact 1 Stabilization The utility crossings approved in Action ID: SAW-2006-32163 shall begin within 30 days of a new DA authorization. Erosion control measures shall be in place prior to beginning said work and shall be maintained until the ditch bank is stabilized with permanent vegetative cover. These erosion control measures shall include: Silt fence shall be installed along the top of slope of portions of the ditch adjacent to any land disturbance. Silt fence shall be inspected after any runoff producing rainfall events and cleaned or repaired as needed. The silt fence shall be maintained until the ditch bank is stabilized with permanent vegetative cover. This work shall be completed prior to March 24, 2010 as it does not require USACOE concurrence and is part of the approved erosion control plan for this project. Rock check dams shall be installed in the ditch immediately upstream and downstream of the utility crossing. The check dams shall consist of clean aggregate material and be constructed in a manner which will allow for continued flow of water during dry periods. The check dams shall be inspected after any runoff producing rainfall events and cleaned or repaired as needed. The check dams shall be maintained until the ditch bank is stabilized with permanent vegetative cover. At this time, they shall be removed with care so as not to disturb underlying soils and root mat. Note that these check dams are already in place and are understood to constitute a temporary fill of wetlands and will require USACOE concurrence. No additional work, aside from maintenance with hand held equipment as needed, shall be performed until a issuance of a permit. Any additional work, including removal, shall be performed only at the written direction of the USACOE. Upon completion of utility work, disturbed portions of the ditch bank and invert shall be reconstructed with 3:1 side slopes and a 2 foot wide bottom. This area shall be sodded with bermudagrass and monitored to insure that erosion is not occurring and that permanent vegetation is established. Sod shall also be placed in the footprint of check dams once they are removed. Adjacent disturbed areas shall be seeded per the schedule. Any disturbed areas with grades in excess of 5:1 that are not provided with rip rap shall be sodded with bermudagrass. Drainaize Structure Retrofit Work associated with retrofitting an old, dysfunctional drainage structure located at the rear of lot 21 shall begin within 30 days of a new DA authorization. At present, the collapsed structure and its outfall pipe are serving as a line sink, drawing down groundwater and sediment and discharging it into the adjacent creek. The scope of work to remedy this situation includes: The existing outfall pipe shall be sealed off to eliminate any further sedimentation at the outlet. Once sealed off, the outlet structure and associated sinkhole shall be backfilled. A new concrete outlet structure with a cast iron frame and grate shall be installed in the vicinity of the old box. The rim elevation shall be set just above existing grade, at the approximate elevation of the old box. A 15" RCP-shall be installed between the new box and a new concrete FES located in the creek. An additional drop inlet will be installed at the end of a swale between lots 21 and 22. This swale currently directs water from the adjacent yards down a washed out area leading to the creek and is creating additional erosion problems. Finally, a junction box will be installed prior to the FES to reduce flow velocities and optimize the pipe alignment at the discharge point. Erosion control measures shall be in. place prior to beginning said work and shall be maintained until the area is stabilized with permanent vegetative cover. These erosion control measures shall include: Silt fence shall be installed along the tops of slopes of areas adjacent to any land disturbance that receive runoff and above the proposed flared end section. Silt fence shall be inspected after any runoff producing rainfall events and cleaned or repaired as needed. The silt fence shall be maintained until the area is stabilized with permanent vegetative cover. Hardware cloth and gravel type filters shall be installed around the proposed drop inlets. They shall be inspected after any runoff producing rainfall events and cleaned or repaired as needed until the area is stabilized with permanent vegetative cover. A rip rap apron shall be placed at the outfall of the proposed 15" RCP in the flow line of the creek. Rip rap shall also be utilized to reestablish the 2:1 creek bank in the vicinity of the outfall pipe. This apron is proposed as permanent erosion control measure. Upon completion of work, all areas shall be seeded per the schedule. Any disturbed areas with grades in excess of 5:1 that are not provided with rip rap shall be sodded with bermudagrass.
