5. For 5I/In-situ groundwater remediation reports- Do a quick review of report to see if

Conclusions/Recommendation Section, and monitoring data tables. Compare pre -
injection levels of sulfate, iron; phosphate, etc. to post injection levels and note this
below. If there are no impacts evident, indicate `No impacts from injection evident'.
NOTE: The groundwater quality at the site must be monitored until concentrations meet
pre -injection levels or 2L standards before the permit can eventually be Completed in
BIMS, and a Completion Letter Sent to Permittee.
For Non -Discharge reports- Also check influent and effluent concentrations, most
permits require a minimum 95% reduction in concentrations.

Executive Summary
On behalf of Trex Properties, Innovative Environmental Technologies, Inc. (IET) has prepared the
following injection report. This report has been prepared to document remediation activities conducted
at the site.
The injection program was implemented from January 71h to 171h 2019. The remedial approach
employed the oxidation remediation product "Ferric Oxide activated Persulfate". This product pairs a
self -activating oxidation persulfate chemistry while providing the appropriate components for a longer
lasting biological attenuation process following the initial oxidation event.
The materials introduced across 19 of 20 injection wells included 158,858 pounds of sodium persulfate,
13,826 pounds of ferric oxide powder in approximately 69,536 gallons of solution. Although several
attempts were made IW-16 was not installed due to obstacles encountered prior to reaching the desired
depth. Surfacing of remedial material was encountered during the implementation of the design at
several of the Injection Wells; namely (IW-1, IW-2, IW-3, IW-6, IW-7, IW-10, IW-11 and IW-13).
Ultimately full dosages were applied to IW-3, IW-6 and IW-7 using decreased flow and pressure and
revisiting the location multiple times. IW-1 and IW-2 were not completed in full, but 7 direct push
locations were installed in the area around these two locations in order to distribute the material via
injection. IW-10, IW-11 and IW-13 did not receive a full dosage but IW-12 received the materials left
over from these wells. IW-12 is immediately upgradient of the uninstalled IW-16 location and 10, 11
and 13 were not accepting any more volume without excessive daylighting occurring in and along the
road (Cullman Ave). IW-12 shares a screened interval with IW-13 so the extra material was added there.
The point by point dosages along with instances of surfacing and other field notes are noted in the
attached field logs.
Introduction
The Cullman Ave site located in Charlotte, NC, was identified as having soils and groundwater impacted
by the historical release of chlorinated hydrocarbons. An injection program was conducted by IET for
the remediation of CVOC impacts in both soil and groundwater throughout the designated areas utilizing
advanced oxidation technologies.

IET injected a total of 19 injection wells and 7 direct push locations at the Cullman Ave. site. Details
regarding each injection attempt are provided in the field logs in the Appendix.

Direct -Push -Driven Perforated Rod
Placement
A Powerprobe 9500 unit comparable to the one
pictured in Figure 1, was utilized to drive the injection
screen (as seen in Figure 2) to depths ranging from 5
to 27 feet below ground surface (bgs). It is at these
depths, that the likelihood exists of being in close
proximity to the targeted waters in the more
permeable layers that were identified during

Subsurface Pathway Development
Compressed air was used to propel all injectants ,
into the subsurface. Compressed air was first
injected into the subsurface at approximately IMMe
150 pounds per square inch (psi) until a
significant pressure drop was observed at the
injection pressure vessel. This process is referred
to as pathway development. The intent of this
step is to open pathways in the subsurface for FIGURE 2: RETRACTABLE INJECTION SCREEN
the injectants to follow. These pathways are
believed to be those more permeable pathways along which VOCs are more likely to have
migrated, both in the vadose and saturated zones. Liquid and liquid -entrained injectants are then
delivered with pressurized gas to the pathways that were produced during the development step.
Liquid Injection
A predetermined volume of the sodium persulfate and ferric oxide material was immediately
injected into the subsurface fractures and voids that were developed during the compressed air
injection step.
Liquid Rinse
A small amount of water was injected to clear the injection lines and sand packs of the injectant.
Post Injection Line Purge
Compressed air was injected to clear the lines of all material and to force the remedial components
further into the formation before moving to the next injection location.

Conclusion
The injection program at the Cullman Ave. site was implemented based on the proposed design with
slight field modifications. All proposed injectants were introduced at the targeted depths. It is expected
that the injection event will yield positive results and IET looks forward the assessing the data following
future sampling events.
