Please find attached five (5) copies of the modified Pre-Construction Notification (PCN) Form for the
above referenced project. As I briefly discussed with you on the phone, this project was approved as a
roadway project under Permit No. DWQ# 06-0138. The bridge for this project over the Greensboro
Urban Loop was originally to be constructed under the Urban Loop project which was permitted under a
separate permit for the Urban Loop project. Due to timing and contractor commitments, the bridge was
not built as part of the Urban Loop project and was put into the Hornaday Road project. Bent 3 of the
bridge will impact the buffers for the recently relocated Long Branch. The permanent buffer impacts will
be the footprint of the three (3) bridge columns extending out of the ground and will include 4 sf of
impacts to Zone 1 and 26 sf of impacts to Zone 3. The temporary impacts to construct this bent are 2,553
sf in Zone 1, 2,635 sf in Zone 2 and 2,370 sf in Zone 3.
There was a field meeting held on March 31, 2010 with the City of Greensboro, Dane Construction and
Sue Homewood and Amy Euliss with NC DWQ. I have attached a copy of the minutes from that
meeting. A preformed scour hole is located near the bent and will be relocated upon completion of the
bent construction.
I have also included five (5) copies of the revised title sheet and plan sheet showing these additional
buffer impacts.

In response to your requests, Amy Euliss and Sue Homewood, NC Division of Water Quality (DWQ)
meet with Mr. Cordell and other City of Greensboro staff, a NCDOT Division 7 Inspector, and Adam
Holcomb of Dane Construction regarding the placement of bent ##3 on March 31, 2010. As a follow up
to our meeting, Dane Construction sent us a letter dated March 31, 2010 detailing the construction
activities that will occur adjacent to and inside DOT's Long Branch Mitigation Site and the Randleman
Lake Buffer. Through issuance of this letter, we approve the commencement of construction activities
and placement of sediment and erosion control activities in the Randleman Lake Buffer as detailed in the
March 31, 2010 letter from Dane Construction, as long as the following conditions are met:
1. DWQ permit no. 20060138 shall be modified to account for permanent and temporary impacts
to Randleman buffer associated with bent placement and sediment and erosion control measures.
An modification application shall be received within 30 days of your receipt of this letter.
2. Any disturbed vegetation shall be replanted with like vegetation.
3. If the Long Branch mitigation site is inadvertently harmed, this site shall be repaired. This could
result in increased monitoring time of the mitigation site. If any repairs to the mitigation site are
necessary, these shall be coordinated with and approved by NCDOT and DWQ.
4. Please reassess the location of the preformed scour hole. Diffuse flow must be achieved at the
outlet of the preformed scour hole. If necessary, the placement of the scour hole might need to
be shifted away from the bridge bent.

The Hornaday Road widening project includes a bridge over the Greensboro Urban Loop. The original permit (DWQ# 06-
0138) for this project only included the Hornaday Road roadway portion. The Hornaday Road bridge was proposed to be
constructed as part of the Urban Loop project which was permitted under a separate permit as well.
This modification is for buffer impacts to construct an interior bridge pier in the buffers of Long Branch.
