EXPRESS REVIEW ACCEPTANCE LETTER

On April 5, 2007, the Wetlands/401 Unit of the Division of Water Quality received a Fax/email request
from Mr. Jay Guffey ofDavis-Martin-Powell and Associates regarding a project known as "Marvin Ridge
Middle and High School-stormwater management plan" for acceptance into the Express Review
Program. This letter advises you that your project. will be accepted into the Express Review Program
once the following items are received:
a. The application fee of $1500.00 made payable to the North Carolina Division of Water
Quality [stormwater management plan with five drainage areas];
b. Three (3) complete and collated copies of the Express Review Stormwater Management Plan
Application [http•I/h2o enr state nc.uslncwetlands/documents/Exp Rev SWMA.pdf];
c. Three (3) copies of all supporting information pertaining to this project (site development
plans, stormwater management plans, worksheets, etc.);
d. One (1) original and two (2) copies of an Operation & Maintenance Agreement for the
stormwater management facilities.
