REQUEST FOR MORE INFORMATION
Dear Mr. Perry:
On December 7, 2006, the Division of Water Quality (DWQ) received your application dated December
6, 2006 to impact 0.04 acres of 404/wetland, 100 linear feet of intermittent stream, and 3,300 square feet
of Zone 1 Neuse River basin protected riparian buffer, and 2,200 square feet of Zone 2 Neuse River basin
protected riparian buffer, to construct the proposed residential subdivision. On February 26, 2007, the
DWQ received additional information from you, however, the DWQ has determined that your application
remains incomplete and/or provided inaccurate information as discussed below. The DWQ will require
five copies of the additional information in order to process your application to impact protected wetlands
and/or streams on the subject property. Therefore, unless we receive the additional information requested
below, we will have to move toward denial of your application as required by 15A NCAC 2H .0506 and
will place this project on hold as incomplete until we receive this additional information. Please provide
the following information so that we may continue to review your project.
Additional Information Requested:
1. As previously requested, please provide documentation from NC DWQ or NC Forest Resources
indicating that the stream proposed to be impacted was determined to be intermittent.
2. The design for level spreader no. 3 looks acceptable; however, please submit a DWQ level
spreader worksheet for the redesigned level spreader. The level spreader worksheet is available

3. Please explain why the drainage area for level spreader No. 1 has changed from 6.4 acres
(reported in your Jan. 16, 2007 submittal) to 1.6 acres (reported in your Feb. 26, 2007 submittal).
Please provide a level spreader design detail and worksheet for level spreader No. 1 that is based
on the correct drainage area.

Please contact the DWQ within three weeks of the date of this letter to verify that you have received this
letter and to provide us with a date that we may expect to receive this requested information. If we do not
receive this requested information within one week of the date you indicate it will be forthcoming, your
project will be withdrawn and you will need to reapply with a new application and a new fee. Please
contact me in writing and Lauren Cobb of the DWQ Raleigh Regional Office. If we do not hear from you
within three weeks, we will assume that you no longer want to pursue this project and we will consider
the project as withdrawn.
This letter only addresses the application review and does not authorize any impacts to wetlands, waters
or protected buffers. Please be aware that any impacts requested within your application are not
