On May 4, 2009 the Division of Water Quality (DWQ) received the application sets dated May 3, 2010
for the above referenced project. The DWQ has determined that your application was incomplete and/or
provided inaccurate information as discussed below. Please provide five (5) copies of the following
and/or fee so that we may review and approve your project.
Additional Information Requested:
® PCN failed to fully complete stream impacts section on page 5 of 11. The PCN only reflected
a total stream impact of 85 linear feet but not the type of impact, stream name, perennial or
intermittent information.
® The PCN also shows the Buffer impact box checked C la; yet no buffer impacts are noted in
the PCN.
® This project will require a permitting fee of $240 based on the reading of Water Quality
Certification 3821 (NW39).
Please submit this information or otherwise address this request within 30 calendar days of the date of this
letter. If we do not receive this requested information within 30 calendar days of the date of this letter, your
project will be withdrawn and you will need to reapply with a new application and a new fee.
This letter only addresses the application review and does not authorize any impacts to wetlands, waters or
protected buffers. Please be aware that any impacts requested within your application are not authorized (at
this time) by the DWQ. Please call me at 919-733-1786 if you have any questions regarding or would like
