Division personnel have reviewed and approved your request to transfer ownership of the subject permit,
received on January 5, 2005. This permit modification documents the change in ownership.
Please find enclosed the revised permit All other terms and conditions contained in the original permit remain
unchanged and in fW1 effect This permit modification is issued under the requirements of North Carolina General
Statutes 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental
Protection Agency.

In compliance with the provision of North Carolina General Statute 143-215.1, other lawful standards and
regulations promulgated and adopted by the North Carolina Environmental Management Commission, and the
Federal Water Pollution Control Act, as amended,
is hereby authorized to discharge wastewater from a facility located at the

ITC �Iighway 135 south of Stone�vi.11e
Rockingham County
to receiving waters designated as an unnamed tributary to Buffalo Creek in the Roanoke River Basin in
accordance with efltuent limitations, monitoring requirements, and other conditions set forth in Parts
II III and IV hereof.
This permit sha11 become effective Apri115, 2005.
This permit and authorization to discharge shall expire at midnight on April 30, 2007.

Continue to operate an existing 0.005 MGD wastewater treatment facility with the
following components.

+ Sandfilters
+ Chlorine disinfection
4 Dechlonnation unit
This facility is located off NC Highway 135 south of Stoneville at the Jose's Restaurant
WWrP in Rockingham County.
2. Discharge from said treatment works at the location specified on the attached map into
an unnamed tributary to Buffalo Creek, classified WS-IV waters in the Roanoke River
Basin.
