Ms. Martin - the Division received confirmation that the subject facility no longer discharges
wastewater to waters of the state. NPDES permit NC0044199 is hereby rescinded, effective
immediately. This message is to close the permit file for the Division, and for your records.
If in the future your facility wishes to discharge wastewater to the State's surface waters, it
must first apply for and receive a new NPDES permit.
If you have any questions concerning this matter, simply reply to this message.
PLEASE NOTE: as part of the Division's conversion to paperless communication, this email is the
only notification you will be sent regarding the permit rescission. No hard copy will be mailed.

The facility is no longer connect and has been filled in (final grading and sowing still need to be completed on my last
visit); however, I am waiting on some final information from James & James (requesting documentation of the closure
process for our records) before giving the Ok on the rescission.

In regards to Sam's Mart, the closure took place on September 30th, 2020.
We had our lead technician on-site to oversee the pumping of the plant.
Before pumping Josh Ogle (maintenance lead) had removed the fence,
motors, blowers, electrical, all plumbing, and metal grating. Haywood
Electric was called in to remove the service line to the plant.  Mike's Septic
handled the pumping of the station and dumped at MSD. Once the plant
was pumped out by Mike's Septic we had C&G Grading deliver 108 cubic
yards of gravel/sand mix which was packed into the plant, 2 loads of fill dirt,
and one load of topsoil which is still in process of being spread due to the
weather.
