On January 27, 2010, March 5, 2010, and April 22, 2010, the DWQ received additional information from you, however, the DWQ has determined that your application remains incomplete and/or provided inaccurate information as discussed below. The DWQ will require additional information in order to process your application to impact protected wetlands and/or streams on the subject property. If we do not receive the requested information, your project will be formally returned as incomplete. Please provide the following information so that we may continue to review your project. Additional Information Requested: Please correct the following issues on the profiles of the aluminum and concrete outlet structures: a. Per your Supplement Form and plan view, the pond must be excavated to elevation 123.00. In the outlet profiles, however, it appears that the pond is excavated only to elevation 127.60. b. The side slopes must be shown on the profile: 10:1 for the vegetated shelf and 3 elsewhere. c. The table states that the value for A is 18.3 feet. This does not appear to be correct. e. Please provide a value for H. 2. In the Wet Detention Basin Supplement Form, please provide the correct value of the elevation on top of the additional storage volume. This value will be different that the temporary pool elevation of 133.50 fmsl. Please be certain to provide 1.0 foot of freeboard above this level. 3. Please clarify the planting plan to state that the size of all plants will be four cubic inch containers. 4. Please correct the sediment removal depth in the wet detention. In the main treatment area, this value will be equivalent to the permanent pool elevation minus the sediment clean-out elevation. In the forebay, this value will be equivalent to the permanent pool elevation minus the elevation where 25% of the forebay volume is comprised of sediment (see 5.5 of the BMP Manual for more information). 5. You must provide the Required Items Checklist associated with the Level Spreader Supplement Form and all required items listed therein at the correct scales. The checklist includes a signed and notarized Operation and Maintenance Agreement for the Level Spreader. 6. Please re-design the level spreader such that there is enough space for the level spreader device. Please show all proposed contour lines downslope of the level spreader. The filter strip will need to be graded if a uniform, transverse slope does not currently exist on the ground. Feel free to contact Annette Lucas at annette.lucas@ncdenr.gov or 919-715- 3425 if you need to discuss the design of the level spreader-filter strip further. Please provide your calculation of the flow rate of stormwater that will be directed to the level spreader. 8. Please provide a soils report that is stamped by an appropriate professional to determine the level of the SI-I JVT based uii an in-field soil Luring taken witi?ili the footprint of the proposed wet detention basin. 9. The drainage easement around the filter strip (and all other BMPs and BMP components) must include an additional ten feet around its perimeter. Please submit this information within 30 calendar days of the date of this letter. If we do not receive this requested information within 30 calendar days of the date of this letter, your project will be withdrawn and you will need to reapply with a new application and a new fee.
