The attached project narrative has more detail regarding the scope of this project and the
reason for the permit modification request, but briefly, the Town of WiIsons Mills has
completed construction on this project and is requesting its permit for the project to be
modified to reflect the constructed scope of work. The permit should show approximately
30,417 LF of 8-inch PVC gravity sewer and approximately 3,979 LF of 8-inch DI gravity
sewer as being constructed,and approximately 60,975 gallons per day of new wastewater
being discharged to Johnston County's wastewater system.

PROJECT DESCRIPTION
This project constructed new gravity sewer mains for the Town of Wilson's Mills in Johnston County,
NC. The Division of Water Resources issued the permit for this project March 30, 2017, as permit No.
WQ0039122. Per the original permit, the work to be completed comprised the following:
"...approximately 34,300 LF 8-inch PVC gravity sewer and approximately 3,630 LF 8-inch DI gravity
sewer, a 100-gallon per minute pump station...as well as approximately 1,691 LF 4-inch PVC force
main...and the discharge of 68,175 gallons per day of collected domestic wastewater into the Central
Johnston County Regional Wastewater Treatment Facility's existing sewerage system..." However,
during construction, several components of the permitted work were modified and or removed at the
Town's request. As such, this permit modification application is submitted to show the scope of the work
constructed which now requires Division of Water Resources review and approval.
As noted on the Fast Track Application,the scope of work to be permitted is as follows:
• Approximately 30,417 LF 8-inch PVC gravity sewer(SDR35 and C900)
• Approximately 3,979 LF 8-inch DI gravity sewer
• Discharge of 68,175 gallons per day of collected domestic wastewater into the Central Johnston
County Regional Wastewater Treatment Facility's existing sewerage system
The 100 gpm pump station and its associated 4-inch PVC force main was removed from the project and
should no longer be included in the permitted work.
It should be noted that in the Fast Track Application, Section IX Setbacks and Separations, Questions I
and 2 were answered "no", and Question 6 was answered "yes" reasoning for these answers shown
below:
• Question I was answered"no"because there are multiple locations where a final vertical grade of
36" is not met. To ensure these areas meet regulation I5A NCAC 02T .0305(g), ductile iron pipe
has been used at each installation where the distance between the top of the pipe and final grade is
less than 36 inches.
• Question 2 was answered "no" because there are multiple segments of sewer that cross through
existing wetlands. Any sewer segments that cross through existing wetlands have been installed
with C900 PVC and watertight manholes, which meets standards established by regulation I5A
NCAC 02T.0305(g).
• Question 6 was answered "yes"because there is an aerial sewer segment that was installed. This
segment is supported by concrete support piers on both sides of the installation. The length of the
aerial sewer segment is approximately 61 linear feet; it is composed of 8"ductile iron with a 16"
steel encasement; and it is located at a crossing of Poplar Branch, approximately 1,000 LF
southeast of Pear Tree Lane on the north side of Wilson's Mills Road. Per I5A NCAC 02T
.0403(g)(5),Johnston County and/or Wilson's Mills Public Utilities staff will inspect this line at a
minimum of every six months
It should also be noted that the FTSE form included in this submittal is from the original submittal - per
email conversation with Jason Robinson on July 6, 2020,the original version should be used as the permit
modification is being done in response to a change in project scope.

Thanks for checking in Peter. Theoretically we'd want the flow tracking sheet to be updated for 2020. However, since
this is only being done in response to an inaccurate EC, and also because the shut-down may make it more difficult and
timely to obtain Flow Tracking sheets,we'd be willing to accept the mod request without flow-tracking sheets(you may
need to remind of us this when we begin reviewing the mod app).

Quick question for you as we're working on finalizing the Wilson's Mills permit modification to be resubmitted to DWR—
do you need us to include Flow Tracking forms for this application?The original permit application included multiple
FTSE forms for the different pump stations to which the proposed sewer would flow, and none of the final destinations
changed in the actual construction process.As such,are the original FTSE forms still usable,or do we need to update the
FTSE forms for 2020 flow numbers?

I was going through my"to-do" list and realized I may have never responded to this email. Sorry about that.
Fast-Track permits don't have an expiration date, so if a partial certification is submitted, there really isn't a timeframe
for when a final would have to be submitted. With that said, our Central Office has recently begun going back to any
permit over five years old that Final ECs have not been submitted and following-up on those. They may start doing it for
more recently issued permits.

and keep the permit open for the next phase of construction or certify as Final and modify the permit to reflect the

Ah gotcha,that makes sense. I guess that's where the"including..." in 1.7 of the permit is not all-inclusive, but rather
provides the typical cases as reasons for non-compliance.We'll keep that in mind for the future.
Again, as far as we know, the project will be completed in the future, likely when the funding scenario is right for the
Town,and maybe before then if the need is there.So if we revise the certification form to show partial rather than final,
and specify for which portion of the approved permit the certification covers(in the comment section below Don's seal),
you can approve it and the Town will be good to go?

Thanks for the response.
Any increase or decrease of 10%requires a modification. We've been given this instruction from our Permit Coordinator
in the Central office.
However, if there are plans to finish the entire project in the future, then a "partial" certification can be submitted. The
Certification Form has a box to check for"Partial", and an explanation should be put in the comment section at the
bottom of the form. Once the total project is completed, a "Final" will need to be submitted, and a full set of as-builts
will need to be submitted at that time.
If that's the route you'd like to go, we'll need a new certification form that has"Partial" checked, and it should be
described in the comments section . (If I recall correctly, "Final" was checked on the form you submitted. However, I'm
having trouble locating it now at home, I think I brought it back into the office).

thought I'd just shoot you a quick email. I'm working from home too,so I know the process of getting voice mails
remotely and responding to them is a pain.
Wanted to follow up with you on this conversation about the Wilson's Mills sewer project. We're hoping to understand
what exactly you need to see to approve the permit certification, and we're wondering why we would need to submit a
modification.We were under the impression that a permit modification is required only if the change is an increase in
length/pumping, not a decrease.As far as we know, the plan is to construct the infrastructure that was removed from
this construction when the appropriate funding is available—is it possible for you to approve a partial certification on
the already-constructed improvements such that the Town does not have to reapply to complete these improvements
down the road?

Don,
As described in Standard Condition 1.7 of the permit, a permit modification is required if any changes result in non-
compliance with the permit(pipe length changes>10%, removal of PS and FMs,etc). This should be done prior to
certification, as you are certifying that the project was built within substantial compliance of the permit,which it was
not. Currently the active permit doesn't reflect what was actually installed.

To remedy this,we'll need a new fast-track application requesting a modification (with supporting documentation and
fee). The application should include everything that installed as part of the project (the lengths you listed). The
application should not include anything that wasn't installed (PS & FM).
We'll then issue a modified permit, and we'll need an Engineering Certification submitted for that new modified permit.

Sorry for the confusion.
Sheet 3.01 had to be changed due to alignment changes by NCDOT. A Bulletin Drawing BD-01 was issued (follows 3.01).
Sheet 3.04—Daniel Dr. was removed from the project due to no requests for sewer services on this street.This gravity
was not constructed as noted.
Sheets 5.01 thru 5.04, 6.01 and E-1.01-E-1.04 were not funded and not part of the construction contract.These sheets
were not constructed as noted.
The following lengths of gravity sewer were installed:
8-inch Ductile Iron gravity sewer-3,979 LF
8-inch PVC gravity sewer—30,417 LF
The pump station, force main and associated gravity sewer for that station were not installed as noted above. If you
need me to mark up the certification accordingly I can do so.

We're just getting a chance to review the Final Engineering Certification for the referenced project that you submitted
back in October(sorry for the delay).
The permit that was issued in March 2017 approved 37,930 LF of 8" Gravity line,a 100-GPM Duplex Pump Station, and
1,691 feet of 4" FM.
Some of the record drawing plans say"Not in Contract"and are crossed-out(Page C-3.01);some sheets say"Eliminated
From Project" (Sheet C-3.04);some of the plan sheets say"Not Constructed" (Pages C-5.01 through E-1.04). This seems
to include the PS and Forcemain. Were they not constructed?
We need to know what was installed and if it is approximately what was described in the permit. Was the PS and FM
not constructed? What are the lengths of the gravity and forcemain that were installed?
