Here are the answers to your questions:
1. Project # 9B.208511
2. All are occurring at the stream crossing. There was a separate permit issued previously for the small tributary
work.
3. Yes only the crossing will be dewatered under this permit. We plan to just place a dike upstream. We will seal off
the lower side at the existing pipe. We do not want to impact the pool at the outlet end of the pipe.
4. The rip rap is covered under the 3 and 27 to my understanding.

I've spoke with Kent in the past regarding this project, but I noticed Stephanie's name was listed as the contact, so I'm
emailing you both. I have a few questions about the permit application for Flat Shoals Rd.
1. Is there a project number? NW 27's require written concurrence, so I'll need a number to debit or a check for
the permit application fee.
2. Are all of the impacts occurring at the stream crossing, or are some occurring on the other trib just upstream on
the other side of the road? I remember some discussion about how to stabilize a portion of this trib. Please
specify which impact is where.
3. I'm assuming only the crossing will be dewatered. Please specify. Are the impervious dikes upstream and
downstream of the pipe, equaling loft of temporary impacts, or is it only five feet as indicated on the permit
application.
4. Is John issuing a NW13 for bank stabilization, or is all of the rip rap covered under the 3 and 27?
