This form must be completed by Permit Writers for all expedited permits which do not require
full Fact Sheets. Expedited permits are generally simple 100% domestics (e.g., schools, mobile
home parks, etc.) that can be administratively renewed with minor changes. These permits can
include facilities with more complex issues (Special Conditions, 303(d) listed water, toxicity
testing, instream monitoring, compliance concerns).

Mr. Konsul,
I have been reassigned the finalization of the permit for the Queens Harbor facility. The public notice
process is completed and we are getting close to permit issuances. I just wanted to touch base with you
about transmittal of the final permit.
In order to provide more convenience, control, and security to our permittees and assist them in
processing their transactions, The Division of Water Resources is currently transitioning towards
electronic correspondence. This will hopefully provide more efficient service to our permittees and
other partners and will allow us to more effectively process and track documents. We are writing to ask
you for your approval of the transmittal of documents related to your permitting and related activities
with the Division in an electronic format. Documents will be emailed to the appropriate contact
person(s) in your organization in a PDF format.

Concur with the issuance of this permit provided the facility is operated and maintained
properly, the stated effluent limits are met prior to discharge, and the discharge does not
contravene the designated water quality standards.
Concurs with issuance of the above permit,
I have no �pareciable expertise in implementation of the Clean Water Act r�,irements
and have minimal understanding of the information provided. As a result, I have no
basis for opposing the permit.

How's it going up there Bradley?
The only things that I saw were in the permit description - dechlorination was spelled wrong (was spelled
dichlorination) and a backup generator also needs to be added to the permit description.
I didn't see any other issues.

Hey Wes,
Just wanted to touch base with you on this permit renewal. Kristin Litzenberger was originally working
on this renewal. With her move over to another group I was reassigned the renewal last week. Kristin
had already drafted a permit for public notice back in April. Apparently there were some issues getting
the paper to notice it and it looks like it finally went to notice on August 20th. So, this one is out now
for notice and will be closing in mid -September.
In BIMS it indicates that a draft was forwarded to the regional office for comment. I didn't see anything
in the online files indicating a response from the region. so, I wanted to check in to make sure you had
seen the draft and see if you have any comments of concerns? The draft permit and cover letter are in
Laserfiche in the 2020 folder. The application is there as well in the 2019 folder.
Please let me know if you are okay with this one or have comments. If you want to talk about it then let
me know and we can set up a time to discuss.
