Dear Mr. Clarke:
On April 9, 2010,the Division of Water Quality(DWQ)received your revised application dated March 31,
2010,to fill or otherwise impact 0.001 acres of 404/wetland, and 90 linear feet of perennial stream,to provide
two additional entrances into the site. On April 9, 2010,the DWQ received additional information from you,
however,the DWQ has determined that your application remains incomplete and/or provided inaccurate
information as discussed below. The DWQ will require additional information in order to process your
application to impact protected wetlands and/or streams on the subject property. Therefore,unless we receive
five copies of the additional information requested below,we will place this project on hold as incomplete until
we receive this additional information..If we do not receive the requested information,your project will be
formally returned as incomplete. Please provide the following information so that we may continue to review
your project.
Additional Information Requested:
1. Please re-submit your site plans(with the appropriate storm water BMPs located)on full plan sheets at
a scale of no smaller than 1"=50' with topographic contours shown. Additionally, please submit one
(1)data CD of full size plans in TIFF Group 4 format(black and white,not grayscale or color). If the
plans are too large to store in TIFF format,they can be stored in PDF.
2. As previously requested in our March 19,2010,Request For More Information correspondence to you,
per the requirements of GC3705,this project is subject to Stormwater Management Plan (SMP)
Requirements for Applicants Other Than the North Carolina Department of Transportation(see
http://portal.ncdenr.or web/wq/swp/ws/401/certsandpermits/swmanagement). For each proposed
BMP, please provide a completed BMP Supplement Form with all required items(see see
http://portal.ncdenr.org/web/wq/ws/su/bmp-manual). Your proposed SMP is not adequate to remove 85
percent Total Suspended Solids(TSS)from you site. Please revise your SMP accordingly and resubmit
it.
