Dear Mr. Partrick:
On April 6, 2010, Sue Homewood and Amy Euliss, Division of Water Quality (Division) regional
office staff visually inspected the Hornaday Road Extension project with Thomas Cordell and
David Rudd, City of Greensboro staff, and Adam Holcomb, Dane Construction project manager
and other contractor representatives. An inspection was conducted of the site to determine
compliance with 401 General Certification Authorization #06-0138 issued to the City of
Greensboro. At the time of inspection the project limits were in compliance with the limits of the
authorization. However it was determined that Stream #3 was not identified as a stream on the
erosion control plans and therefore had not been properly protected during land clearing and
.grubbing.
Please note that Water Quality 401 Certification (DWQ No. 06-0138) issued to the City on July 3,
2008 had the following two conditions that were not met:
The permittee shall conduct a pre-construction meeting with all relevant parties and
regulatory agencies prior to commencing work on any portion of this project. A copy of the
final construction drawings shall be furnished to NCDWQ prior to, or during, the pre-
construction meeting. The permittee shall ensure that the final construction drawings comply
with the permit drawings contained in the application and any amendments and modifications
subsequently submitted to DWQ and approved with this certification. Any deviations from
the approved drawings are not authorized unless approved by the NC Division of Water
Quality.
The outside buffer, wetland or water boundary located within the construction corridor
approved by this authorization shall be clearly marked by highly visible fencing prior to any
land disturbing activities. Impacts to areas within the fencing are prohibited unless otherwise
authorized by this certification.
Adherence to these Conditions would have likely prevented the current situation and properly
protected Stream #3. The current conditions now pose an additional threat for stream impacts
from continued construction. It is essential that the activities in this area of the project be
carefully monitored for impacts downstream of the project boundaries. Any turbidity and/or

sediment leaving the site shall be noted immediately and reported to Sue Homewood within 24
hours.
The Division also conducted a Construction Stormwater Inspection (DWQ Permit NCGO10000).
The Stormwater permit requires you to inspect erosion control measures and stormwater outfalls
at least once every seven days and within 24 hours after any storm event of greater than '/Z inch of
rain in a 24-hour period. Sufficient records are currently being maintained for this project. The
Division commends you for attention to these permit requirements.
