This CoC is issued pursuant to the requirements of North Carolina General Statue 143-215 .1 and the Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated October 15, 2007 (or as subsequently amended). If any parts, measurement frequencies or sampling requirements contained in this general permit are unacceptable to you, you have the right to request an individual permit by submitting an individual permit application. Unless such demand is made, the certificate of coverage shall be final and binding. Please note that this CoC is not transferable except after notice to the Division. The Division may require modification or revocation and reissuance of the CoC. Acting Secretary January 17, 2017 Subject: Compliance Evaluation Report Former Johnny's Save -A -Sum Ground Water Remediation NPDES Permit NGC510462 Cabarrus County On January 12, 2017, Edward Watson of the of the Mooresville Regional Office (MRO) completed a site inspection of the remediation system at the Former Johnny's Save -A -Sum facility located at 3970 Hwy 601 S. Concord, NC 28025. Enclosed is a copy of the inspection report. The system is online and performing at levels consistent with the active permit for this facility. Please submit a copy of the recent waste manifest for the spent activated carbon units along with recent groundwater analytical data. The continued monitoring of this facility is recommended. There are continued concerns regarding securing site access to the public. Permit requirements state that the treatment system which includes the recovery wells are to be properly protected. Truck. traffic through the property has caused damage to the recovery and monitoring wells. As. a response to this report, please provide a plan to prevent truck traffic from using the site as a cut through to Flowes Store Road. The system appeared to be in good operating condition. The flow meter was changed in mid -June of 2016 and is in compliance with the permit. Additionally, the filter socks had been changed from 25ug particulate flow size to 50ug to help avoid clogging of the filter bags. The air stripper was cleaned out in June of last year and the system efficiency seems to have improved as lab reports displays a reduction of BTEX and MTBE in the effluent samples. Recent effluent samples have been collected and the sampling results will be submited to DWR upon receipt from the lab. The system carbon vessels were recently changed and the DWR has requested- a copy of the waste manifest to be submitted via email. A 55 gallon drum was present in the area of the oil and water separator during the previous inspection. This barrel has been removed. Discussion regardinq the installation of ballards at the drivewav enterances that could be spaced far enough apart so a semi trucks would not be able to pass through. There is also evidence that personel vechicals are also using this site as a pass through as evidence by the tire marks present turning onto the site from Hwv 601 Dassina around the temporary barricade curently in place. It was emphasized that protection of the onsite wells and system were part of the NPDES permit. It was also mentioned that if wells needed to be replaced or repaired as a result of this traffic it would be done at a cost to the RP. During the visit the following items were reviewed. 1. System operations. During the site visit the system was operational and we were able to test the.recovery wells for flow. 2. Effluent flow measurements. With.the installation of the'new flow meter effluent flow.is now able to be measured. 3. Flow meter calibration records A new calibrated flow meter has been installed. 4. System effluent sampling records. Laboratory sample analysis data was provided by the. project manager to indicate system operations and effectiveness. 5. Monitoring and recovery well conditions. There are still issues with the condition of the recovery wells and at least two of the on -site monitoring wells. The permit states the system. is to be maintained and protected from damage. The recovery wells are considered to.be a part of"the permitted groundwater remediation system. All wells are subject to the maintenance requirements of 2C .0112. Recovery well # 4 was reviewed during the site visit and there was evidence of the well gallery box collapsing due to heavy truck weight: Many of the site monitoring and recovery well have become covered by mud and many of these wells are not able to be secured to do damage. -The permit indicates the wells are to be maintained. However, this does not appear to be happening.Overall the site is in poor condition. The oil and water separator has a 55 gallon drum in place to capture free product. However, the drum has holes in the bottom at the base and is not capable of retaining liquid. The system oil and water separator is not being used for its designed purpose. Rather it is being utilized as a sediment settling tank. Water from the Oil and Water Separator is transferred to a 275 gallon holding tank. Water in the holding tank is transfer by way of a transfer pump to a 50 micron filter bag to collect suspended fines. After the filter sock, water enters the Air -stripper. The water leaving the air -stripper is sent to a pair of in -line GAC cylinders and finally, trough another filter sock before being discharged to an unnamed tributary of the Irish Buffalo Creek, a class C stream that eventually -enters the Yadkin -Pee Dee basin. The system outfall was observed and not foam, odor or solids were viewed in'the outfall water. Sampling can be conducted from Influent and effluent are in place for the treatment system. Several issues of concern were present during the initial site visit. The following issues have been adressed: No sampling events of the treatment unit performed since 12/13/2012. Recent lab analysis has been received on 6/21/2016 by the MRO. No reocrd of flow meter calibration. A new flow metered was installed on 6/20/2016. Treatment system is in poor condition and not operational. System has been repaired and is currently operational. Monitoring and recovery wells are in poor condition. This circumsatnce still remains. However, once the site has been secured, work can begin to repair MWs and RWs will no longer be subject to having heavy trucks traveling over them. PM was advised to contact MRO if the system is down for any reason, including maintenance. Filter bags were allowed to be increased in micron size to help reduce system maintainence. Manifest for spent carbon has been requested to be able to track carbon cradle to grave. An Annual Report is not required for this system. According to the NOD, there was one primary potential deficiency as follows: 1. Laboratory - "The effluent is to be sampled quarterly for the parameters listed in NCG510000. Permit was renewed in October 2011 and all parameters are to be reported for the first eight sampling events before a reduction can be allowed." Our response to this item is presented below: 1. Regarding the effluent sampling, as required in the NCG51000 permit, sampling of the effluent should be conducted quarterly. In reviewing the data over the past three years (2009 - present), effluent samples were collected all four quarters in 2009, during three quarters of the year in 2010 and 2011, and have been collected three of the four quarters required so far in 2012 (the next sampling event is scheduled for Decembers 2012) and the effluent limitations of the permit were not exceeded. Based on the review of the operation and maintenance forms, the samples were not collected in December 2010 because they were inadvertently missed and were not collected in September 2011 because the system was not operating at the time of the visit. Total phenols were not analyzed in the effluent samples likely because it was a constituent that, according to the permit, could be discontinued if there were no detections. Shield sampled for total phenols during the October 2012 sampling event and will continue to include it in the sampling unless there are no detections above the practical quantitation level (PQL) for the first eight sampling events based on the permit effective October 2011. Phenols were added to the table submitted in the November 5, 2012 Active Remediation Monitoring Report (ARMR). Shield also measured pH during the October 2012 sampling event and will continue to include it in the quarterly.sampling events, and it will be included in the tables in future ARMRs. We have reviewed the influent and effluent data for the pump and treat system from August 2001 through October 2012. The pump and treat portion of the system is optimally operating in compliance with the permit. Several pipes were broken, however, we do not know the extent of the damage yet. Someone from Shield Engineering was contacted by the Fire Department across the road from the system and Shield personnel visited the site. The main power breaker has been removed and the system is not operating at this time. A police report was filed with the Concord Police Department. The police report incident number is 13-00261. Shield does not have a copy of the police report at this time. We will contact you once we know what repairs will need to be done and approximately when we expect the system to be operating again.
