The geometric mean is equivalent to the antilog of the arithmetic mean of the logarithms of the indi- vidual values. For purposes of calculating the geometric mean, values of zero (0) shall be considered to be one (1). PART II A. MANAGEMENT REQUIREMENTS 1. Change in Discharge All discharges authorized herein shall be consistent with the terms and conditions of this permit. The discharge of any pollutant identified in this permit more frequently than or at a level in excess of that authorized shall constitute a violation of the permit. Any anticipated facility expansions, production increases, or -process modifications which will result in new, different, or increased discharges of pollutants must be reported by submission of a new NPDES application or, if such changes will not violate the effluent limitations specified in this permit, by notice to the DEM of such changes. Following such notice, the permit may be modified to specify and limit any pollutants not previously limited. 2. Facilities Operation The permittee shall at all times maintain in good working order and operate as efficiently as possible all treatment or control facili- ties or systems installed or used by the permittee to achieve com- pliance with the terms and conditions of this permit. 3. Bypassing Any diversion from or bypass of facilities necessary to maintain com- pliance with the terms and conditions of this permit is prohibited, except (i) where unavoidable to prevent loss of life or severe property damage, or (ii) where excessive storm drainage or runoff would damage any facilities necessary for compliance with the effluent limitations and prohibitions of this permit. The permittee shall promptly notify the Water Quality Section of DEM in writing of each such diversion or bypass. 5. Removed Substances Solids, sludges, filter backwash, or other pollutants removed in the course of treatment or control of wastewaters shall be disposed of in a manner such as to prevent any pollutant from such material from entering waters of the State or navigable waters of the United States. PART II 6. Power Failures In order to maintain compliance with the effluent limitations and prohibitions of this permit, the permittee shall either: a. Provide an alternative power source sufficient to operate the wastewater control facilities; or, if such alternative power source is not in existence, b. Location of Nearest Dwelling: The nearest dwelling is approximately 500 feet from the wastewater treatment plant site. 10. Receiving Stream or Affected Surface Waters: Unnamed tributary to Horton Branch a.- Classification: C b.' River Basin and Subbasin No.: Yadkin 03-07-12 Page Two C. Describe receiving stream features and pertinent downstream uses: The receiving stream contained a small amount of flow on this inspection date. Minnows and small fish were noticed in the stream. The proposed discharge location in the unnamed tributary is approximately 500 feet upstream of the confluence with Horton Branch, which contained an excellent flow. There are no other discharges known to occur in the unnamed tributary. PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS 1. Type of wastewater: 100o Domestic 0% Industrial a. Special monitoring requests: No 3. Additional effluent limits requests: No 4. Other: No PART IV - EVALUATION AND RECOMMENDATIONS The applicant, James Gibson, proposes to discharge wastewater generated from a two bedroom single family home into an unnamed tributary to. Horton Branch. .a Page Three At the proposed discharge location there appeared to be very low flow; later confirmed as having 7Q10 of zero (0) after completing the Waste Load Allocation procedures for a single family discharge. Approximately 500 feet downstream of the proposed discharge location the receiving stream emptied into Horton Branch which appeared to have a positive 7Q10 flow. If the discharge were relocated to this stream then secondary limits would probably apply. The applicant was explained the problems of having a 7Q10 of zero at the present discharge location and that in order for a permit to be issued the discharge would have to enter a positive flow stream. Therefore, a new discharge location may be forthcoming. This Office recommends that the permit be denied at the present discharge location proposed in the application. w .
