III. Certification Statement:
I ins certify to the best of my knowledge that the addition of
the volume of wastewater to be permitted in this project has been evaluated along the route to the receiving
wastewater treatment facility and that the flow from this project is not anticipated to cause any capacity
related sanitary sewer overflows or overburden any downstream pump station en route to the receiving
treatment plant under normal circumstances, given the implementation of the pIanned improvements
identified in the planning assessment where applicable. This analysis has been performed in accordance
with local established policies and procedures using the best available data. This certification applies to
those items listed above in Sections I and II plus all attached planning assessment addendums for which I
am the responsible party. Signature of this form certifies that the receiving collection system or treatment
works has adequate capacity to transport and treat the proposed new wastewater.

Lee Street Athletic Complex Project -Benson, NC
Dear Sir or Madam,
Enclosed please find Fast Track Sewer Permitting materials, documents, and other information for the Lee Street
Athletic Complex. The project includes two new baseball fields, parking, and additional green space for outdoor
recreational league sports and is owned and developed by the Town of Benson. A new field house will be
constructed to serve the facility.
Sewer is to be provided to a new field house via a lift station and sewer force main. The force main will pump
effluent to an existing gravity manhole located to the south of the project site in the Mann Street travel way.
Effluent will enter the lift station from a single 4 inch diameter gravity sewer service from the field house. Note
that we have submitted a variance application to Ms. Christyn Fertenbaugh for relief on a few elements associated
with the lift station. In our discussions with her, she had asked us to go ahead and submit the permit application
so the variance could be written into the approval document.

There are several applications ahead of this one but we'll get to it asap. We have not heard from Christyn on the
variance request. We'll need comments from her prior to issuing the permit.
A cursory review of the application shows we need the following:
Item V must be fully completed. We need the downstream sewer permit number and system wide collection system
permit number.

Thanks Ted. No issues on our end with renaming it. Note that we have submitted a variance with Christyn Fertenbaugh
for a few lift station items. She should be close to wrapping up our approval and asked to go ahead and submit the
permit dots to your office so she could write the approval into permit language.

We received the subject application on 8-10-20. There is a stormwater permit issued to the Town with this exact same
project name. To differentiate, we will add 'Sewer' after the project name. The name of the project is modified to "Lee
Street Athletic Complex Improvements —Sewer".

Hi Ted,
Sorry for the delay in getting back to you. I have updated Item V with the system wide collection system permit
number.
We couldn't find any records of the existing sewer permit number as it was installed many years ago.
On item X.1, I marked that no on account of our variance requirement. I'll follow up with Christyn regarding our
variance request to see if we can get this one buttoned up.

There are several applications ahead of this one but we'll get to it asap. We have not heard from Christyn on the
variance request. We'll need comments from her prior to issuing the permit.
A cursory review of the application shows we need the following:
Item V must be fully completed. We need the downstream sewer permit number and system wide collection system
permit number.

Item U was marked "no". Was this marked 'no' because the variance is needed?
