Please have the facility submit a Rescission Request for this permit to the Central Office, and our Regional Office will
confirm. Then we can proceed with rescinding the Certificate of Coverage. Mike Meloy or Suzanne McCoy may

Was not sure who to address this to so please let me know if there is someone else I should include.

Pilot (PFJ Southeast) has a stormwater general permit for the oil water separator discharge at this site (NCG080896).
Pilot took control of the site several years ago and transferred the permit over from Wilco and Apex continued with the
sampling and DMR submittal.
Sampling was always conducted directly from the effluent chamber of the OWS as no one onsite or at Pilot knew where
the line discharged.
Due to an exceedance of TSS reported on the Jan -June 2020 DMR, Apex recommended to Pilot that we clean the OWS
system and flush all lines to meet DEQ limits.
During the system cleaning, Apex observed that the OWS actually discharges to the onsite sanitary lift station.
As such Apex reported this to the county utility, who has informed Apex that they were aware of this discharge which
occurred prior to Pilot acquisition of the site.
Based on these findings we will need to cancel the existing general permit.
Please let me know what you need from Pilot/Apex to cancel the permit at this time.
